Child #1 has 4 medications on site at the facility (glucose tablets, insulin, Baqsimi, and Diazepam). There are periods of time in which the facility staff may need to administer the medication. The facility does not have completed medication logs on file nor signed parental consent. Requirement: A parent shall provide written consent for administration of medication or a special diet. An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered. Correction: Medication logs were given to family to complete. A fan was observed mounted on the wall, and the cord was dangling down within reach of the children. (CORRECTED ON SITE). Dusty vents were observed in PKC1, PKC4, PKC6, and PKC7. Sharp adult scissors were observed within reach of the children in PKC2. (CORRECTED ON SITE) PKC4 - The trash can and the wall behind the trash can were visibly soiled. PKC8 - An area of the metal baseboard was separating. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Vents were cleaned on 11/15. Scissors were put out of reach on 11/15. Trash can and wall were cleaned on 11/27. Baseboard was fixed on 11/27. The fan was taken down the day of inspection. A container and a large container of hand sanitizer were observed within reach of children in an unlocked closet. PKC2 - Two mops with attached cleaning solutions were observed stored in areas accessible to the children. The containers with cleaning solutions were not labeled. PKC4 - Hand sanitizer, and adult medications were observed in the backpack that was stored within reach of the children. PKC6 - A container of cleaning solution was observed in the bathroom. The container was not labeled. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: Toxic materials will be put in a locked cabinet. This includes hand sanitizer and toxic cleaning supplies. Mops will only have soap and water in them and be labeled. Adult medications will not be in the emergency backpack. Facility self-reported the following incident: Staff persons #1 and #2 were taking a group of preschool aged children to the playground at approximately 2:05pm on 10/8/24. During the transition to the playground, child #1 was able to slip away from the group and was left behind in the building when the group went to the playground. Child #1 was found in the hallway by another staff person. The staff person returned the child to the group on the playground, and reported being back in the office by 2:10pm. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff will have primary care groups. Each staff will have a lanyard with supervision cards. Staff will review their cards during the transition of pre-k counts and wrap care.
