On 9/5/25 facility self-reported non-compliance. On 9/2/25 Staff # 1 and # 2 were supervising 16 Pre K Counts children. Staff # 1 took 1 child to the bathroom, and 5 children followed Staff # 1 out of the classroom and went to the pick-up area downstairs in the school. Staff # 2 was unaware that 5 children were unaccounted for until Staff # 1 came back into the room with 1 child. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Tiered LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The Legal Entity must create a supervision policy. This supervision policy must address supervision during transition times, such as visiting the bathroom and dismissal times. The legal entity must receive approval of the policy from the Regional Office. Once approved by the Regional Office, the legal entity must review policy with all staff and have staff sign off that the understand and will follow the supervision policy. The operator shall provide a correction date for when the policy will be completed. Correction: 9/5/2025 Children will be supervised at all times. 9/4/2025 Reviewed update dismissal policy with all staff to ensure consistency with dismissal procedures and supervision of children. 9/8/2025-10/3/2025 All staff had coaching and modeling sessions to ensure proper supervision during transitions. 9/18/2025 Staff comleted 2 hour Supervision Transition training. 10/1/2025 Director comleted 2 hour Supervision Transition training. 10/9/2025 All staff reviewed LVCC Supervision policy 2.05 Supervision of Children.
