Clean, age appropriate (3270.106(a))

Two rest mats in the Pre-K room were observed to be ripped, foam was exposed. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: The ripped mats were removed from childcare. Inspection: Renewal. Emergency plan (3270.27(a)(5))

Facility Emergency Plan did not contain a plan that provides accommodations for infant, toddles, children with disabilities and children with chronic medical conditions. Requirement: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. Correction: The emergency plan with accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions. Inspection: Renewal. Building Surfaces (3270.76)

Sprinkler head detached in the attic, causing minor water damage. Ceiling tiles and baseboards in the entrance and beginning hallway were observed. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Sprinkler system immediately corrected/fixed. All water damage has been repaired- Ceiling tiles, baseboards. walls were replaced/fixed. Building Surfaces (3270.76)

Baseboard in the children's play areas was observed to have areas of peeling paint. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Infant sleep position (3270.119)

Two infants were observed asleep in cribs with a pacifier strap attached to their pacifier. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.
