Building Surfaces (3270.76)

At the time of renewal inspection on 10/17/25 observed signs of water leakage on the ceiling tiles in Toddler room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The affected ceiling tiles were inspected to access the extent of water damage. The leak was found. Repairs were completed. All stained ceiling tiles were cleaned. Inspection: Renewal. At the time of the inspection on 3/18/25 observed the rug in Pre-K Teddy Bears room and the rug near the bookshelf in Pre-K 4 Owls room were stained and in need of cleaning. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The rug in Pre-K Teddy Bears was replaced om new one. The rug in Pre-K Owls room was replaced on new one. Inspection: Allocated Unannounced Monitoring. No peeling paint or plaster (3270.77(a))

At the time of the inspection on 3/18/25, two spots of peeling paint were observed in the hallway near Room #2. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: All peelings were painted in the hallway near room #2. Restraints prohibited (3270.113(e))

At the time of the inspection on 3/18/25, observed the video footage shot at nap time on 3/12/25, child #1 lying on their mat and staff person #2 sitting on the bean bag next to the child. Per staff #2, in order to comfort the child, staff person #2 put their legs across the child torso area appearing to prevent the child from getting up for approximately 15 minutes. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: All staff attended Staff Meeting. Agenda: Supervision during nap time. We discussed such topics as: understanding the issue, alternative approaches to supporting restful sleep, role-playing scenarios and discussion, staff commitment and accountability. At the time of the inspection on 3/18/25, file of staff person #1 did not include acceptable verification that education had been evaluated to determine US equivalency. Requirement: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. At the time of renewal inspection, file of staff person #1 did not include acceptable verification that education had been evaluated to determine US equivalency and qualify staff person#1 for the position of an assistant group supervisor. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.
