At the time of renewal inspection on 10/17/25 observed signs of water leakage on the ceiling tiles in Toddler room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The affected ceiling tiles were inspected to access the extent of water damage. The leak was found. Repairs were completed. All stained ceiling tiles were cleaned. Inspection: Renewal. At the time of the inspection on 3/18/25, observed the video footage shot at nap time on 3/12/25, child #1 lying on their mat and staff person #2 sitting on the bean bag next to the child. Per staff #2, in order to comfort the child, staff person #2 put their legs across the child torso area appearing to prevent the child from getting up for approximately 15 minutes. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: All staff attended Staff Meeting. Agenda: Supervision during nap time. We discussed such topics as: understanding the issue, alternative approaches to supporting restful sleep, role-playing scenarios and discussion, staff commitment and accountability.
