At the time of the investigation, it was confirmed that Staff Person #1 physically restrained Child #1 by placing both hands on the child to keep him on the sleeping mat during nap time. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: Provider has retrained all staff on rest time expectations through rest time expectations policy review, reviewing scenarios rest time expectations during April 20, 2026 staff meeting and individual conversations. 2 hour diaper check (3270.135(e))

At the time of inspection on 12/19/25, written diaper changing record was reviewed. According to the written record, child #1 diaper was not checked for more than 3.5 hours. Child #1 diaper was checked at 7:01 AM and then at 10:47 AM. Requirement: A staff person shall check a child's diaper at least every 2 hours and whenever the child indicates discomfort or exhibits behavior that suggests a soiled diaper. A staff person shall change a child's diaper when the diaper is soiled. Correction: The provider will check at least every 2 hours, before going down for rest time, and whenever the child indicate discomfort or exhibit behavior that suggests a soiled diaper. During the renewal inspection conducted on 10/03/2025, it was noted that the emergency contact forms for children #1, #2, and #3 had not been updated within the required six-month period. The updated forms were missing from both the master file and the childcare space. Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Health and Safety Training/Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)/3270.31(f)(10))

During the renewal inspection conducted on 10/03/2025, it was noted that staff person #1 has completed Pediatric first aid and CPR training, however, the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. At the time of renewal inspection observed that fire drills document did not include hypothetical locations of the fire for each drill. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that the hypothetical locations of the fire are rotated around the facility for each drill such that the hypothetical location is never the same for consecutive drills. Correction: Provider will conduct fire drill and will ensure that the hypothetical locations of the fire are rotated around facility for each drill, so the location of hypothetical fire is in a different location each drill. Clean and good repair (3270.102(a))

At the time of renewal inspection observed that rest mats were ripped on the edges exposing foam in Butterflies room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Comply with CPSL/CPSL information (3270.32(a)/3270.192(4))

At the time of renewal inspection, file of staff person #1 did not include current DHS approved FBI clearance. Staff #1 had FBI PDE clearance on file that cannot be accepted. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff person #1 may not work in a child care position at the facility. Correction: Staff person #1 will not work in a childcare position until DHS required FBI clearance will be provided.
