At the time of the inspection, according to self-reported non-compliance, staff #1 left child #1 unattended in Pre-KB Classroom 8 while child #1 was in the bathroom. At that time class transitioned to the playground. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Admin will inspect rooms before classes leave classroom as well as playground. At the time of the inspection on 2/10/25 video footage from 1/21/25 was reviewed and observed that staff person #1 was lifting multiple children by one arm and carrying them across the room while their legs were dangling in the air. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Had staff #1 into office to go over an action plan that we put into place. She is being monitored throughout the day. We also have her on a training plan of training around handling and naturing of toddlers. At the time of inspection on 2/10/25, in the refrigerator located in the kitchen observed two trays of burnt rice leftovers that was prepared and had to be served on 2/6/25. Requirement: Food stored, prepared or served shall be clean, wholesome, free from spoilage, free from adulteration and safe for human consumption. Correction: Kitchen has been cleaned down and cleaning schedule has been made. No food is being put back into the refrigerator. At the time of inspection observed dirty toys in Room #8 (houses), #10 (toys and pillows), #7, #5 (toys, stuffed toys/dolls), #6 (toys in the kitchen and dolls), #4 (musical instruments), #2 (cars, big Lego blocks). Sink located in Room #9 had a peeling material that covered the sink. Diaper plastic cabinet located in Room #7 was broken and dirty, shelfs were dirty with residue of old tape. Shelves in Room #6 were dirty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Cleaning scheduled have been placed in classrooms for weekly cleaning. Toys that are larger have been put on a weekly schedule to be cleaned and sanitized in kitchen.
