During the complaint investigation conducted 6/10/26, it was found that 6 out of 16 infants were sleeping in cribs without at least 2 feet of space required on three sides. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: during the visit we shifted the cribs a few inches back to their designated spaces. We reviewed the tape areas on the ground with the teachers and reinforced the tape. During the complaint investigation conducted 6/10/26, there were 16 children (the youngest child in the group was an infant age child) present in the infant classroom with staff person # 1, # 2, # 3, # 4, and # 5, which exceeds the maximum group size for an infant. Requirement: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. Correction: At any one time, the small play area will have no more than 4 infants. the larger play area will have no more than 8 infants in that group. During the renewal inspection conducted 10/1/25, the health assessment reviewed for child #1 did not include a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease. During the unannounced inspection conducted 7/8/25, three infants were observed sleeping in cribs inside of the infant room childcare space without having at least 2 feet of space between each crib. Four cribs were observed close together in a row with three out of the four cribs being occupied by sleeping infants. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. During the complaint investigation conducted 7/8/25, it was found that child #1 was seen at an urgent care physician's office to receive medical treatment to the foot for an injury that occurred at the facility during a fire evacuation. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. During the complaint investigation conducted 7/8/25, it was found that child #1 was seen at an urgent care physician's office to receive medical treatment for an injury that occurred at the facility during a fire evacuation. The operator did not telephone notice the appropriate regional office within 24 hours. Requirement: The operator shall immediately notify a child's parent and shall telephone notice to the appropriate regional office within 24 hours if a child in care at the facility is hospitalized or receives emergency room treatment. During the complaint investigation conducted 5/7/25, it was found that staff person #1 was observed aggressively approaching staff person #2 and communicating in a harsh manner, in the presence of 5 pre-k children and one young toddler. Requirement: A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: Staff person #1 was immeidately terminated due to their behavior and unprofessionalism because it violates our school policies. Inspection: Complaints- Legal Location. During the complaint investigation conducted 4/8/25, it was found that staff are often on cell phones and not supervising children. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Tablets have been fixed so that staff do not use cellphones for procare. Stairs, exits, etc. unobstructed (3270.91(a))

During the unannounced inspection conducted 4/8/2025, the back hall space exit, before entering the preschool room was observed obstructed with boxes, plastic storage containers, a table, chairs, tarp and a garden wheelbarrow. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: The side hallway was cleaned and organized. Locked or inaccessible (3270.66(a))

During the unannounced inspection conducted 4/8/2025, pot and pan sanitizer, re-Juv-Nal disinfect, and top clean soap were observed on a low shelf in the school age 3rd floor bathroom. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The cleaning materials stored on the unoccupied third floor are now locked in the cabinet. Inspection: Unannounced Monitoring. During the unannounced inspection conducted 4/8/2025, the third-floor school age childcare space was being utilized as a storage space. There were tools, boxes, shelves, large cushions, extra toys and a vacuum cleaner being stored. Requirement: A facility shall provide indoor child care space for individual and group small muscle activity. Facility Person/Symptoms of Disease (3270.153)

During the complaint investigation conducted 4/8/25, It was found that staff have been allowed to work while sick with a contagious illness while the facility was made aware. Requirement: A facility person with symptoms of a communicable disease or infection that can be transmitted directly or indirectly and which may threaten the health of children in care shall be excluded from attendance until the facility operator receives notification from a physician or CRNP that the person is no longer considered a threat to the health of others. The notification shall be retained in the facility person's file. Exclusion from the facility is required for diseases and conditions specified in 28 Pa. Code Chapter 27 (relating to communicable and noncommunicable diseases). Correction: Staff have never been allowed to work with a known contagious illness. Towels labeled and laundered (3270.134(b))

During the complaint investigation conducted 4/8/25, It was found that rags for cleaning surfaces and cleaning children were being used without being labeled. Cloth rags that are used for cleaning surfaces are also being used to wash children's hands and nose. The same rags are used for multiple purposes and are not labeled for use of specific children. Requirement: Cloth towels and washcloths shall be labeled with the child's name, used by only the named child and laundered weekly. The director shall arrange a laundry schedule with the parent. Correction: Cleaning rags have never been used on children or for multi-purpose. The individual face wash cloths for each child will no longer be used. We will only use disposable towels. During the complaint investigation conducted 4/8/25, it was found that, children are often strapped down in child size chairs with t-straps for extended times after am snack, lunch and pm snack. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. During the renewal inspection conducted 9/16/2024 upon request, there was no evidence produced verifying that the facility tested their fire detection system every 30days.
