A complaint inspection was conducted 6/26/26. Through the investigation, it was discovered that Child #1 was injured at the facility on 4/21/26 when their fingers were pinched in the hinged side of the door to the playground when a parent opened the door from inside the building to enter the playground. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: A guard will be installed on the door to eliminate the possibility of a finger getting caught. Inspection: Complaints- Legal Location. Surfaces cleaned (3270.135(b))

A renewal inspection was conducted 9/9/25. At that time, the diaper changing pad in the Freshman Room was observed to be peeling at one end and therefore could not be properly sanitized after each use. Requirement: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering. A renewal inspection was conducted 9/9/25. At that time, A&D Ointment was observed on a floor level shelf in the Sophomore Room, where it was accessible to the toddlers in the room. Requirement: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children. Small Toys and Objects (3270.103)

A renewal inspection was conducted 9/9/25. At that time, diapers in plastic sleeves were observed on a floor level shelf in the Sophomore Room, where they were accessible to the toddlers in the room. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Health and Safety Training (3270.31(f))

A renewal inspection was conducted 9/9/25. At that time, the preservice training was reviewed for all staff. Staff Person #2, who has been employed longer than 90 days, has not completed pediatric first aid and cpr training. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Until such time as the required pediatric first aid and cpr training has been completed, staff person #2 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #2, then staff person #2 may not work in a child-care position at the facility. Correction: Instructor has been scheduled to give pediatric first aid and CPR training to Staff #2. A renewal inspection was conducted 9/9/25. At that time, the preservice training was reviewed for all staff. Staff Person #1 had not completed mandated reporter training prior to the expiration of their previous training. This is evidenced by the mandated reporter training on file dated 6/25/20 and 8/26/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). The file for Staff Person #1 and Staff Person #3 did not include verification of high school education to qualify them as assistant group supervisors. The file did include verification of two years' experience for each staff. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A renewal inspection was conducted 9/9/25. At that time, a water-stained ceiling tile was observed above the sink in the girls bathroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Roofer was already called and on the schedule to repair some roofing shingles and repaint stain in the ceiling tile. Inspection: Renewal. A renewal inspection was conducted 9/18/24. At that time, the preservice training was reviewed for all staff. Staff Person #3, who has been employed longer than 90 days, did not have verification of the health and safety training: Get Started With Center Based Care- Revised 2022 on file at the facility. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Staff person #3 will have until 10/5/24 to complete the required training. Until such time as the required training has been completed, staff person #3 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff available to supervise staff person #3, then staff person #3 may not work in a child-care position at the facility. A renewal inspection was conducted 9/18/24. At that time, the staff files were reviewed. The TB test on file for Staff Person #2 was conducted 5/16/22, more than 12 months prior to their date of hire. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. A renewal inspection was conducted 9/18/24. At that time, the emergency transportation plan was not posted in each child care space. Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. Correction: The emergency transportation plan was updated to include who will accompany a child in the event of an emergency and will be posted in each child care space. Inspection: Renewal. A renewal inspection was conducted 9/18/24. At that time, the trash can in the Junior Room bathroom was not equipped with a lid. There was no trash can in the Senior Room bathrooms. Requirement: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle. A renewal inspection was conducted 9/18/24. At that time, there was nothing preventing children access to the kitchen area, where the stove is located that is used for preparing children's lunches. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.
