There was an epi-pen at the facility that did not identify the name of the child for whom the medication was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Parent provided the original box epi-pen came in with the child's name on it. Inspection: Renewal. There was no prescription for the epi-pen belonging to Child #2. Requirement: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. Correction: I had my parent provide the original box with epi-pen included inside prescription box with instructions for use and child's name included. Parent also provided prescription to use if necessary. I emailed copies to inspector on epi-pen box with instructions. Inspection: Renewal. The Emergency Contact Form for Child #1 lacked the address of the child's physician. Requirement: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care. Correction: I had parent fill in the address on my "Emergency Contact" form to complete the pediatrician's information. I emailed my inspection the completed emergency contact form. Clean, age appropriate (3290.105(a))

Observed that Child #3, an infant, lacked age-appropriate rest equipment at the facility. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program. Correction: I set up a pack n play for infant in my facility and send a picture by email to inspector. I sent a picture of pack n play with child's name on it to my inspector by email. The file for Staff #1 contained a NSOR Certificate that was updated in an interval greater than 60 months. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include records of training required by the Department. Correction: In the year of 2025, if I am still in business of caring for children, I will have my NSOR certificate dated within timeframe of 60 months. Inspection: Renewal. Training regarding plan (3290.24(c))

The files for Staff #1 and #2 contained Emergency Plan Trainings that were dated more than 12 months apart. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training. Correction: I will have my next "Annual Emergency Plan Training and Drill" before June 2026 to stay within my 365 days from the previous one. Inspection: Renewal. Small Toys and Objects (3290.103)

Two Bingo Dabbers were observed in a low unlocked drawer in the Preschool Classroom. Both of these items were labeled, "Choking Hazard, Small Parts," and were accessible to children who are still placing items in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. The file for Child #1, enrolled for more than 6 months, lacked a child service report to provide information to the family about the child's growth and development in the context of the services being provided. Requirement: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e). Prohibit items in crib (3290.105(j))

Multiple toys including teething ring, blocks, and cars were observed in the pack n' play while Child #1 was sleeping. Requirement: Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib. Correction: All toys were removed form the pack n' play. Inspection: Renewal. Locked or inaccessible - in original container (3290.64(a))

Wet Ones Disinfecting Wipes were observed on a low shelf and 4 Bingo Dabbers were observed in a low unlocked drawer in the Preschool Classroom. Both of these items were labeled, "Keep out of Reach of Children," and were accessible to the children in care. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
