At the time of the inspection, staff #2 confirmed child #1-6 were in Prekindergarten B room without supervision. Staff #1 moved child #1-6 and did not confirm them entering into Prekindergarten A room and were observed in the Prekindergarten B room. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Staff #1 was terminated due to the supervision violation. Inspection: Complaints- Legal Location. At the time of the inspection, it was confirmed by the director how staff person #1 harshly and roughly placed the child on the floor and spoke to child #1 in a stern tone being in time out. Requirement: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: Staff #1 was terminated on 9/18/25 based on the violation of state and TLE policies by using a "time out" and by physically placing a child down in a rough manner. At the time of the inspection, the file of staff person #1 had adoption as the purpose of their Pennsylvania state police clearance. Requirement: The operator shall comply with CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #1 -- may not work in a child care position at the facility until completed state police clearance results are obtained. A facility's person record shall include a copy of the completed clearance information required under CPSL. At the time of the inspection, the prekindergarten B had chipped paint behind the bookshelf. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: On 9/16/2025 the facility maintenance repaired the chipped paint wall in the pre-kindergarten B classroom behind the children's bookshelf Inspection: Renewal. At the time of the inspection, the file of child #1 did not include the address of the individual designated by the parent to whom the child may be released. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: The child's parents, at pick up time were requested to, and completed filling in the addresses of the persons to whom the child may be released Inspection: Renewal. 2 feet apart (3270.106(f))

At the time of the inspection, rooms 130, 104, 115 and prekindergarten had cots that were observed with less than 2 feet of space on three sides of the cot. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Each staff member was re-trained on nap time procedures, focusing specifically on the spacing of cots, maintaining 2 feet of space on three sides. Inspection: Renewal. At the time of the inspection, staff person #1 flicked children, forcibly pushed a pacifier into the child's mouth and cursed in the presence of children. This was confirmed by staff person #2. Requirement: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: Facility person #1's employment was suspended during the time of the investigation and is now terminated, effective today-6/26/25. At the time of the inspection, staff person #3, #4, and #5 were unable to name the children in their assigned group. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: The staff were provided with laminated cards for the classroom and wipe off markers, so they are able to keep track of the children that they are primarily responsible for everyday. Where staff person #1 and #2 were positioned, it did not allow full supervision of children while setting up meals for lunch and cots for nap time in the classroom. During lapse in supervision the child sustained scratches. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Erika Arroy and Autumn Peden completed a supervision training. Supervision-"Where Do I Stand" on the Better Kid Care website. Autumn had already completed this training, so she was not able to get a current date on the certificate-she reviewed the content and completed the training again. During my inspection I observed that the staff 1, 2, did not know whom they were responsibly for. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff members, 1 and 2, completed additional supervision training "Infant Toddler Care: Quality Supervision" from the Better Kid Care training website. The training reviewed the importance of knowing the children who each staff is primarily responsible for. The use of primary caregivers and the importance of accounting for all children was also reviewed with all staff. Inspection: Unannounced Monitoring. During my investigation, I found that staff 1, hit a child during a diaper changing process. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: The employment of staff 1 has been terminated, and staff 1 is no longer a member of the staff in any capacity at The Learning Experience in Levittown. Correction: Staff persons #2 and #3 did an additional training on the "Better Kid Care" site on supervision which featured counting, knowing how many children are in your care, and knowing which children each staff member is primarily responsible for at all times. Handwashing signs (3270.82(h))

At the time of inspection, in preschool room 2B, the handwashing sign present did not indicate when to wash hands. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. At the time of inspection, damaged paint was observed in twaddler room B on the right-side wall towards entrance and in the bathroom. Correction: The Twaddler B room was painted on the right side wall towards the entrance and the bathroom was painted. Inspection: Renewal. At the time of inspection, the water temperature in preschool room 2B measured at 117 degrees. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: The hot water temperature has been adjusted to be below 110 Degrees F. It was measured to be sure that it was at an appropriate level. Inspection: Renewal. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #-1 may not work in a childcare position at the facility. Correction: Provider will obtain all required information for state police record.
