Observed there was no medication log established for a prescription diaper cream or an epi pen in the older infant room. Requirement: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered. Correction: Self-correcting. The child for which the EpiPen was needed for no longer attends Burkett's Watch Me Grow - Bedford. The EpiPen was returned to the child's parent. The last day the child attended was 8/15/25. The diaper cream was thrown away at the request of the parent when they were contacted by the Director as it was no longer needed. Inspection: Renewal. Observed in the cherries preschool room, a water play table which was not in use on the day of inspection, and had water still in it due to not being thoroughly emptied after it was last used. Requirement: A water play table or a container used for water play that contains unfiltered water shall be emptied daily. Correction: Director will retrain staff on DHS requirements for a container used for water play in that it should be emptied daily. Inspection: Renewal. Observed in the older infant room, there was not 2 feet of space on three sides of the pack n plays which were in use. The pack n plays had bags of toys on the sides. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Director will review violation with staff and retrain staff on proper placing of pack n plays while in use. Staff will place pack n plays at required DHS spacing while in use.
