An infant child was observed to be sleeping in a bouncy seat that was sitting inside the crib on the crib mattress. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. Correction: Talking with teachers about the requirements of cribs, cots, beds, etc. That nothing should be in a crib while an infant is in the crib. Inspection: Unannounced Monitoring. 2 feet apart (3270.106(f))

An infant child was observed to be sleeping in a bouncy seat that was sitting inside the crib on the mattress with a 48"X36" art portfolio case acting as a barrier on the inside of the crib side. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Talking with teachers about the requirements of cribs, cots, beds, etc. That nothing should be in a crib while and infant in in the crib. Inspection: Unannounced Monitoring. Small Toys and Objects (3270.103)

An infant child was observed holding a quart sized Ziploc plastic baggie filled with a clear liquid and sensory objects in their hands while sleeping in a bouncy seat that was sitting inside the crib on the crib mattress. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: To talk with the teachers in ITCS about sensory bags, that when children are using them, the must be supervised. Also, that an infant may not use them and cannot be in a bouncy seat in the crib when sleeping child must be removed. Inspection: Unannounced Monitoring. Staff assigned to specific children (3270.113(a)(1))

Staff person #2 was observed leaving the classroom and physical presence of the children to answer the door so a guest could enter. The file for staff person #1 was observed to have a written warning signed and dated by staff person #1, for leaving the classroom and physical presence of the children. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: To re-educate the staff member about the rules of leaving the room unattended. Even though there was a therapist in there. she cannot be counted in the ratio. Health and Safety Training (3270.31(f))

The file for Staff person #1 was observed to lack a valid pediatric first-aid training and pediatric cardiopulmonary resuscitation certificate within 90 days of hire Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: The staff person was not left alone in a room until her CPR card came stating that she now certified in CPR. Her card came on 2/12/2026. A copy of her card was sent to the inspector. No harsh language (3270.113(d))

Based on interviews, Staff #1 told children to "shut up" and "act right" when the children were not listening. Requirement: A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: ALSM placed Staff #1 on administrative leave upon receipt of the complaint on July 9, 2025 and completed an internal investigation to determine if the allegations were founded. The investigation resulted in the accepting of Staff #1's resignation on July 28, 2025. All staff at the Growing Years Center received professional development on the ASLM Children's Services Code of Conduct (COC) the week of August 4, 2025 making them aware of ethical responsibilities of children, families, staff, and the community and consequences for failure to abide by the COC. Stairs, exits, etc. unobstructed (3270.91(a))

Observed the means of egress at the end of the childcare hallway was obstructed with bouncy chairs, strollers, and car seats. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Age and Training (3270.31(e)(4)(i))

Observed staff file #1 contained documentation of training in pediatric first-aid and pediatric cardiopulmonary resuscitation (CPR) which was obtained after the current certificate had expired. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: Pediatric first aid and CPR training dates for all team members will be tracked by the Center Supervisor and the Program Assistant. The current dates will separated by months when renewal is needed, and the Program Assistant will review this information monthly and shared with the Center Supervisor. Arrangements will be made at least two to three months in advance with a certified trainer to offer the needed pediatric first aid/CPR class for the employees that need their certification renewed. Observed staff file #1 contains a child abuse clearance which was dated more than 60 months ago. Observed staff file #2, who began employment after 2/1/25, contained the disclosure form which was replaced, and the correct disclosure form was not signed and in the staff file. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # -1 and 2- may not work in a childcare position at the facility. Correction: Staff person #1 has a PA Child Abuse History Certification that was updated on 5.6.2025, and the current clearance is in the team member's file. Staff person #2 has received the 2025 Disclosure Statement, read and reviewed the form, and signed the correct disclosure statement on 7.31.2025. Inspection: Renewal. Contaminated trash in closed plastic-lined receptacle (3270.67(d))

Observed in the gross motor room, a trash can used to collect trash from lunch was not in a closed lidded trash can. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Emergency Numbers Posted (3270.74)

Observed the phone in the ITCS room did not have the emergency numbers posted by the telephone. Requirement: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility. Correction: The emergency phone numbers list has been moved from the drawer in the ITCS room and is now posted on top of the cabinet by the telephone. Inspection: Renewal. Observed the first-aid kits in the 2 Pre-K rooms were on low shelves and accessible to children. Requirement: A first-aid kit must be inaccessible to children. Building Surfaces (3270.76)

Observed an area of the second pre k room which was disorganized and cluttered and unable to be used as child care. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The second Pre-K room was decluttered, and the materials were all put back in place. The room is now organized and free of clutter, and the children are able to use all the space in the classroom again. Inspection: Renewal. Observed an infant sleeping in a pack n play did not have 2 feet of space on three sides. There were toys and a bouncy chair next to the pack n play. Correction: The play yard in the middle of the room was removed to all the toys and bouncy seat to be moved to an appropriate location. Observed child files #1, #2, and #4 did not contain a child service report conducted within the previous 6 months. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: The Ages & Stages Questionnaire has been completed for children #1, #2 and #4, and have been shared with the families. The completed forms are in the appropriate children's files at the Growing Years Center. The COR Advantage assessments, the tool associated with the High Scope curriculum, have been completed for the children as well. The assessment reports will be made available to the families. Observed the emergency contact form for child file #3 did not contain the health insurance policy number. Observed child file #2, a young toddler, contains a most recent health assessment which is dated more than 7 months ago. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Correction: The parent was contacted, and they are getting a copy of child #2's 18-months health assessment from the child's physician. Observed staff file #5 did not contain documentation of an updated health assessment. The current health assessment was dated more than 25 months ago. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Correction: Staff #5 was off on medical leave/short term disability from April 10, 2025 until July 14, 2025. She did get her health assessment prior to her return. Observed child file #4, who has been in attendance for more than 60 days, did not contain a valid initial health report submitted and answering all of the questions on the department approved form. Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. Correction: The health appraisal for child #4 is now in their file at the Growing Years Center and is dated July 22, 2025. Inspection: Renewal. Refrigerator (3270.107)

Observed the refrigerator by the door in the first Pre-K room needed to be cleaned. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The refrigerator has been removed from the room, and is no longer being used. The refrigerator is going to be removed from the facilty. Inspection: Renewal.
