File for Staff # 1 was not in compliance with CPSL regulations. File for Staff # 1 contained State police and Child Abuse clearances over 60 months. State Police clearances dated 4/15/21 and 6/3/26. Child Abuse clearances were dated 4/15/21 and 6/5/26. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Two facility persons present in facility (3270.54(a))

On 5/11/26 Staff # 1 was left alone in the facility with 4 Preschool children while Staff # 2 went to pick up school aged children. Requirement: At least two facility persons shall be present in the facility when two or more children are in care. At a minimum, one of the facility persons shall be a staff person. Tiered LIS: 1. At least two facility persons shall be present in the facility when two or more children are in care. At a minimum, one of the facility persons shall be a staff person. This portion of the plan shall have an immediate correction date. 2. For a period of 2 months, the legal entity must send staff scheduling to Regional Office. The schedule for the upcoming week shall be sent to the Regional Office by close of business on the Friday prior. The operator shall demonstrate the ability to maintain compliance with the regulation during all unannounced inspections conducted by the Department over the next two months. A few of the rest mats were observed to be ripped and the foam was exposed. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: We will be throwing away matts that are ripped with foam expose. The preschool classroom contains two bathrooms. The first bathroom did not contain a lidded trash can. The second bathroom did not have a handwashing sign posted. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle. Correction: A lidded trash can was placed in the 1st bathroom. During an initial inspection on 5/2/25, facility did not have a working fire system.
