At the time of the inspection, the annual emergency plan training for staff #1 was dated 5/20/24, staff #8 annual training was dated 8/6/24 and staff #5 annual training was dated 3/21/25. The file of staff # 2, 3, 6, and 7 annual training dated 11/11/24. The file staff #4 and 9 did not include the annual emergency plan training. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: Ensure all staff trainings are properly filed once completed. Annual training was done on 11/11/25 for all staff, forms were just not secured in files. Fire safety - 1 yr. (3270.31(e)(4)(ii))

At the time of the inspection the file of staff #4, 7, 9, and 10 did not include the fire safety training. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Health and Safety Training (3270.31(f))

At the time of the inspection, the file of staff #3 & 10 did not include part 2 of the health and safety training, The file of staff #5 did not include the update of the health and safety training. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. At the time of the inspection, the file of staff #3 & 4 did not include a high school diploma or or a general educational development certificate. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. At the time of the inspection, the file of staff #9, 10, & 11 did not include the verification of child care experience. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Building Surfaces (3270.76)

At the time of the inspection, dust accumulation was observed on the vent in the young toddler room. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. No peeling paint or plaster (3270.77(a))

At the time of the inspection, peeled paint was observed in the preschool and older toddler room. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. At the time of the inspection, the file of staff #7 included a health assessment that did not include an examination for communicable diseases and the results of that examination. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Protective Outlet Covers 5 yrs. or less (3270.65)

At the time of the inspection, an uncovered outlet was observed in the preschool room. Non-compliance was corrected on site. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Provider placed an outlet cover on the electrical outlet. At the time of the inspection, sunscreen, bug spray, diaper cream and a bleach and water bottle spray was observed in an accessible location to children. Non-compliance was corrected on site. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Provider removed the sunscreen, bug spray, diaper cream and bleach and water spray bottle in a locked cabinet that is inaccessible location to children. Inspection: Renewal. Inaccessible to children (3270.75(b))

At the time of the inspection, the first aid kit was observed in an accessible location to children in the pre-kindergarten room. Non-compliance was corrected on site. On excursions (3270.75(d))

At the time of the inspection, staff person #2 and #3 the first aid kit did not accompany the group while going outside. Requirement: One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at §3270.75(c). Correction: Staff were retrained on the required materials for excursions out of their classrooms. New material bags were provided. Inspection: Renewal. Handwashing signs (3270.82(h))

At the time of the inspection, a handwashing sign was not posted in the bathroom of the pre-kindergarten room. Non-compliance was corrected on site. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Staff assigned to specific children (3270.113(a)(1))

At the time of the inspection, staff person #1 was unable to name the children in her assigned group. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. No physical punishment (3270.113(b))

Video was reviewed in which staff person #1 was observed forcibly grabbing the arm of child #1. Child #1 was seen by physician on same date and diagnosed with nursemaid elbow. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Staff member was placed on 90 day probation and retrained on proper handling of children. Staff member was also required to complete additional online courses (attached) to continue employment and understand center guidelines on handling children with care.
