    At the time of the inspection, the annual emergency plan training for staff #1 was dated 5/20/24, staff #8 annual training was dated 8/6/24 and staff #5 annual training was dated 3/21/25. The file of staff # 2, 3, 6, and 7 annual training dated 11/11/24. The file staff #4 and 9 did not include the annual emergency plan training. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update.
