Regulation requires staff to provide two written letters of reference attesting to the person's suitability to serve as a facility person. Staff #1 was working in a child care role at the time of this inspection and their file did not contain two letters of written reference. Requirement: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person. An unannounced inspection occurred at the facility. A review of staff files was conducted. See the code sheet for the date of hire. Staff #1 was not hired as required by the CPSL. Staff #1 did not receive their NSOR prior to working in a child care position at the facility. Staff #1 did not receive their NSOR until 3/25/26. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff #1 may not work in a child care role with direct contact with children until documentation per the CPSL is received. Correction: Staff's actual hire date was not updated on documentation. Staff really started 3/30/26. Her hire date and actual start date differ. Staff will be removed from child care until clearances are received and will be hired per the CPSL. A renewal inspection occurred at the facility. A review of the facility's emergency plan was conducted. At the time of the inspection, the following piece to the emergency plan was requested for review and the facility was unable to provide the document for review: the plan for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. Requirement: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. Correction: The facility will provide for review the facility's plan for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions for review. Refrigerator (3270.107)

A renewal inspection occurred at the facility. A physical site review was conducted. Regulation requires refrigerators to contain a thermometer to regulate the inside temperature. During this inspection the refrigerators in the school age corner room and the toddler classroom were missing thermometers and the temperature of the refrigerator was unable to be verified. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: Thermometers will be placed in the refrigerators and remain in the refrigerators at all times to ensure the refrigerator i operating at the correct temperature. Inspection: Renewal. A renewal inspection occurred at the facility. A physical site review was conducted. In the outdoor play area, two shelving storage pieces were observed broken. Broken Little Tikes toys/houses were also observed. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Locked or inaccessible (3270.66(a))

A renewal inspection occurred at the facility. A physical site review was conducted. In the school age classroom, paint, borax, cleaners, and a buck of cleaning water were observed accessible to children. In the preschool classroom, Miracle Gro Potting Mix and in the bathroom of this classroom scrub free toilet cleaner were both observed accessible to children. All of these items indicate the need to be kept out of reach of children. During this inspection these areas were observed to have children utilizing the spaces with the toxic items. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: All items with items that indicate the need to keep out of reach of children will be made inaccessible to children immediately. Inspection: Renewal. A renewal inspection occurred at the facility. A physical site review was conducted. In the school age entrance room and the school age corner room, the plate covers to the outlets were missing, exposing wires and the outlet covers were missing. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Staff person #4 has not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): health and safety training. Staff #4 completed both part 1 and 2 of this training on 4/10/25. Requirement: Staff person(s) shall complete professional development in the topics of 3270.31(f)(1 -- 10). Until such time as the required training has been completed, staff person #4 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff available to supervise staff person #4, staff person #4 may not work in a child-care position at the facility. Correction: Staff will complete preservice training within 90 days of their hire date and will be supervised by an appropriate staff until this training is completed. Inspection: Renewal. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Conditions at the facility may not pose a threat to the health or safety of the children. The OCDEL ANNOUNCEMENT C-22-04 requires facility emergency plans to include continuity of operations during and after an emergency that includes continuing daily operations, backing up or retrieving health and other key records/files and managing financial issues such as paying employees and bills during the aftermath of the disaster. Until such time as the required training has been completed, staff person #2 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff available to supervise staff person #2, staff person #2 may not work in a child-care position at the facility. Correction: Staff person #2 health safety training part 1 was not located and was taken again on 4/3/25. Director will verify trainings using DHS Staff Data Inspection tool to verify necessary trainings are completed befor/during hire. Staff person #2 will be supervised until the training is completed.
