At 7:50 AM, 15 children were observed receiving care in the Young Toddler Classroom childcare space that has a designated maximum capacity of 10. Requirement: The capacity established for an indoor space may not be exceeded except in situations described in § 3270.61(h)(1-3) Correction: Additional staff arrived and took children to a different childcare space. Open other classrooms when capacity has been reached for the childcare space to ensure that required capacities are met. Inspection: Unannounced Monitoring. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Correction: Staff #1 is not in a class by herself until 2/7/26 when she will receive her cpr training. She is in a class with an AGS who has all preservice trainings. In the Infant Room, water temperature at the sink in the classroom measured at 121 degrees. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: Maintenance added a mixing valve to sink. The valve is used to keep the water from going above 110 degrees. Inspection: Allocated Unannounced Monitoring. Mixed Age Level (3270.52)

1.9.26: At approximately 7:50 AM two staff were present with 15 children: including 3 Preschool Aged Children, 8 Older Toddlers, and 4 Young Toddlers. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Correction: We have extended all hours of staff to provide more coverage in the AM to cover for kiddos arriving earlier; ensuring ratios are met. Inspection: Complaints- Legal Location. The only fire safety training in the file for staff person #2 was dated more than 12 months after they began working with children. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Staff person #1 had completed First Aid and CPR training, however the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an (AGS) who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Although Staff #2 and #6 had Pediatric First Aid and CPR at the time of the Renewal Inspection it was observed that Staff #2 and #6 had not completed Pediatric First Aid/CPR within 90 days through a Pennsylvania Quality Assurance System (PQAS) approved curriculum and the trainer has not provided documentation that they are PQAS-certified in that training curriculum. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: Staff #2 and Staff #6 both had CPR Training. The file for Staff #2, #3, #6, and #8 contained an invalid Disclosure Statement. The file for Staff #3 lacked a complete State Police Clearance. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #3 may not work in a childcare position at the facility. Correction: All staff members have signed new disclosure forms. They have been sent and are in files. Building Surfaces (3270.76)

In the Outdoor PlaySpace there was a rip in the squishy surface underneath the green airplane toy that posed a tripping hazard to the children in care. In the Multi-Purpose Room along the right-side wall the baseboard was loose and detached from the wall. In the Multi-Purpose Room, there were three ceiling tiles that were not in good repair as they each had a hole in the section of tile. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Baseboard fixed 7/11/25 Ceiling tiles repaired 7/20/25 Squishy Playground mater is to be repaired by playground company by 7/25/25. Inspection: Renewal. In the Multipurpose Room along the back wall there was damaged plaster and along the right-side wall there was peeling paint. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: All paint and chips have been repaired. Small Toys and Objects (3270.103)

In the Infant Room there was an Infant toy that had individual separate beads that were less than an inch in diameter. In the Outdoor PlaySpace there was mulch, and small rocks in an area where toddlers were playing. The Outdoor PlaySpace also had small pieces of the squishy material that had become loose and posed a choking hazard. These items were accessible to children still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Toy in infant room removed 7/10/25 Mulch and Rocks removed 7/10/25 Rubber will be repaired by 7/25/25 by playground company. Inspection: Renewal. Observed rest equipment in Room 2 and Room 3 that lacked 2 feet of space on three sides while in use. A crib in the Infant Room was observed to lack 2 feet on 3 sides while an infant was sleeping in the crib Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: All staff have been made aware and are now using the two-tile method to ensure that there 2 feet of space on three side of the rest equipment when is use. Inspection: Renewal. Refrigerator (3270.107)

The refrigerator in Room 1 contained a thermometer that was reading 53 degrees. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: We moved thermometer to the back of fridge. Staff assigned to specific children (3270.113(a)(1))

At approximately 10:00 AM, Staff #9 and #10 were observed in the Outdoor PlaySpace with 10 children including 8 Older Toddlers and 2 Preschool Aged Children. Staff#10 named four children as being in their assigned group including 1 Preschool Aged Child, and three Older Toddlers. Staff #9 named five children in their assigned group including 1 Preschool Aged Child and four Older Toddlers, therefore leaving Child #2 not identified as being in an assigned primary group. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff #9 took Child #2 in their primary assigned group. We are also doing ratio checks multiple times a day. Inspection: Renewal. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). In Room 1, there was an albuterol inhaler that did not identify the name of the child for whom the medication was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Names were added to both bags that contain the inhaler. Inspection: Renewal. The file for Staff #7 lacked a Health Assessment completed within the previous 24 months. The file for Staff #4 and #5 contained health assessments that were not conducted within 24 months of the previous assessment. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Mantoux TB (3270.151(c)(2))

The file for Staff #3 contained a tuberculosis screening by the Mantoux method at the time of inspection, but it was observed they did not have tuberculosis screening by the Mantoux method before their first day working in childcare. Requirement: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. Correction: Her start date was corrected. She came in that day to do paperwork with HR and started on Wed of that week with the children. Staff #3 has a current tb screening. Inspection: Renewal. There was no accompanying documentation stating the condition and the risk it might pose to other exposed to this individual.
