Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Written parental consent was missing for an Albuterol Inhaler intended for a child receiving care in the Afterschool 1 classroom. Requirement: A parent shall provide written consent for administration of medication or a special diet. The file for Child #1, an infant, lacked verification that they had received their 2-month Hepatitis B, Rotavirus, DTAP, HIB, Pneumococcal and Polio vaccinations. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Child #1 will be dismissed from care at the close of business on 6.24.25. Correction: The child's parents wrote a letter and signed to inform our center of their decision of not immunizing their child for religious reasons. This letter will be kept in the children's file. A bin was observed to have a cracked area that resulted in a potential pinch point and sharp edges in the Afterschool 1 classroom. School age children were observed to be receiving care in the space and could have access to the first aid kit due to the lack of a lock on the closet and placement of the first aid kit. An unlabeled spray bottle, that was reported to contain a cleaning solution, was observed in the Ones classroom. Toxics shall be stored away from food, food preparation areas and child care spaces. Cleaning materials and sunscreens, labeled Keep Out of Reach of Children, were observed in bins on a shelf above the countertop and a container of disinfecting wipes were observed in an unlocked cabinet, also above the countertop, in the Afterschool 2 classroom. School age children were observed to be receiving care in the classroom, and it was determined that due to their heights the items were accessible to some of the children. Two bins containing sunscreens labeled Keep Out of Reach of Children were observed on a low shelf in the unlocked closet in the Afterschool 1 classroom. The sunscreens could be accessible to preschool children receiving care in the space. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: All cleaning and other toxic materials were moved to an area that is locked and/or made inaccessible to children. Inspection: Renewal. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Wooden fence pickets were observed to be in disrepair causing splintering wood in the outdoor Toddler area play space. One wooden fence picket in the Preschool / School Age area play space was observed to be disconnected allowing access to two screws. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The wooden fence pickets in the toddler area will be fixed to cover the splintering areas so that they area not a harm to the toddlers. The fence picket in the preschool/school age area will be fixed to not have the exposed nails and in proper condition. Inspection: Renewal.
