During the renewal inspection completed on 06/17/26 the last documented emergency drill reviewed was dated 2/11/24. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: A lockdown emergency drill was conducted on 6/18/26 @ 10am. The drill was logged in emergency drill binder. During the renewal inspection completed on 6/17/26 the file of staff person #1, hired (see code sheet) contained a mandated reporter clearance dated 9/15/20 which is more than 60 months old. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. During the renewal inspection the certification representative viewed the staff files staff #2 was missing health and Safety training in the file. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. During the renewal inspection the certification representative reviewed the children's file child #2 was missing a child service report in the file. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: The provider will notify the teacher and request for a copy of the child's child service report and include the copy in the child's file. Inspection: Renewal. During the renewal inspection the certification representative requested to view the annual fire detection documentation but was unable to view it at the time of the inspection and had to be requested from management. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). During the renewal inspection the certification representative reviewed the staff files staff #2 and #3 pediatric first aid and CPR were not PQAS certified. Requirement: Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. During the renewal inspection the certification representative viewed the staff files, staff #1 was missing a health assessment in the file. Requirement: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. During the renewal inspection the certification representative viewed the children files child #2 was missing the release person address on the emergency contact form. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. During the complaint investigation the certification representative observed a sippy cup with no label on it in the infant room. Requirement: Disposable nursers and bottles shall be labeled with the child's name.
