During the renewal inspection the certification representative reviewed the children's files. Child #2 health assessment was missing the statement free from communicable disease. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. During the renewal inspection the certification representative conducted a facility walk through to conducted staff to child ratio. The toddlers were in the outside area. The certification representative observed an old child who appeared to be with her mother supervising the children. The certification representative requested the file of the child after being told the child was a 14. Requirement: A volunteer shall be 16 years of age or older and shall be directly supervised at all times by a staff person. Correction: The provider will notify staff (parent) and request for a copy of their child's latest health assessment and a completed emergency contact form to be included in the file due to the child's age. During the allocated unannounced inspection, the certification representative viewed the staff files staff # 4 was missing a disclosure in the file. Staff #5 was missing a child abuse clearance, FBI results, criminal results, and mandated reporting clearance in the file. Requirement: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #4 and #5 may not work in a childcare position at the facility.
