During complaint inspection on May 13, 2026, staff #1 and #2 named a total of 14 children in their supervision groups, but 18 children were present leaving 4 children unaccounted for, and staff #3, 4 and 5 named a total of 14 children in their supervision groups but 15 children were present, leaving one child unaccounted for. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All teaching staff have been retrained on child supervision requirements, including maintaining constant awareness of the names and whereabouts of each child assigned to the staff. Staff will be able to immediately account for each child in their care. Name-to-face attendance procedures have been implemented and monitored by the Admin TEAM throughout the day. Coaching will be provided to staff to ensure continued adherence to supervision regulations and to prevent reoccurrence of the issue. Inspection: Complaints- Legal Location. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. During complaint investigation on December 19, 2024, it was confirmed that a staff person was observed aggressively grabbing a child. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: After provider was informed of incident, staff was placed on immediate suspension. After completing investigation, staff person was terminated. All active staff completed trainings on mandated reporting, regulating behaviors, and TLE policies.
