During complaint inspection on May 13, 2026, staff #1 and #2 named a total of 14 children in their supervision groups, but 18 children were present leaving 4 children unaccounted for, and staff #3, 4 and 5 named a total of 14 children in their supervision groups but 15 children were present, leaving one child unaccounted for. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All teaching staff have been retrained on child supervision requirements, including maintaining constant awareness of the names and whereabouts of each child assigned to the staff. Staff will be able to immediately account for each child in their care. Name-to-face attendance procedures have been implemented and monitored by the Admin TEAM throughout the day. Coaching will be provided to staff to ensure continued adherence to supervision regulations and to prevent reoccurrence of the issue. Inspection: Complaints- Legal Location. During renewal inspection on July 16, 2025, the record of staff #1 contained a Child Abuse Clearance for volunteer purposes. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility. Correction: Staff #1 will obtain a Child Abuse clearance for employment purposes. Inspection: Renewal. During renewal inspection on July 16, 2025, the health assessments of children #1 and #2 did not include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease or a statement that age-appropriate screenings recommended by the American Academy of Pediatrics were conducted since the time of the previous health report required by this section. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. No physical punishment (3270.113(b))

During complaint investigation on December 19, 2024, it was confirmed that a staff person was observed aggressively grabbing a child. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: After provider was informed of incident, staff was placed on immediate suspension. After completing investigation, staff person was terminated. All active staff completed trainings on mandated reporting, regulating behaviors, and TLE policies.
