During the time of inspection, it was observed that a shelf was broken with manipulatives on them in the Rascals classroom. A basket with magnetic manipulatives was broken in the gingerbread classroom. This was corrected during the inspection. In the Rascals room, Clorox stain remover and cans of disinfectant wipes were observed to be accessible to children in the cabinet under the diapering area. Corrected on site. During the inspection, children were observed to be inadequately engaged by teachers and the curriculum causing pushing, fighting, climbing on tables, throwing toys, and crying for much of the observed time. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: We have transitioned a new teacher into the classroom who has more experience and is better equipped for this specified age group. Inspection: Renewal. During the inspection, staff in the Rascals room were heard threatening to withhold food from a child as a form of discipline. A bottle in the infant room refrigerator was observed to be unlabeled with a child's name. Requirement: Disposable nursers and bottles shall be labeled with the child's name. It was alleged that Staff #1 grabs children, restricts their movements, and yells at them. Allegations include the use of bear hugs to restrict movement and leaving marks and scratches on children. Requirement: Tiered LIS: 1.A facility person may not use any form of physical punishment, including spanking a child. 2.Staff #1 will complete 4 hours of in-person training on early childhood classroom management techniques. Training must be approved by the certification representative prior to registration. 3.Staff #1 will be observed interacting with children in the classroom on a weekly basis. Observations will occur at different times of day and be documented for review by the certification representative. During mealtime, sausage links were observed to be served to young toddlers in the middle nursery. The links were served whole instead of being cut into small pieces, posing a choking hazard. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Teachers will break larger food items into smaller pieces before providing it to their students. Inspection: Unannounced Monitoring. During a complaint investigation, it was confirmed by staff that harsh and loud language was being used in the classroom. Prior to the investigation, leadership held a meeting with Staff #1 about their interactions with children. Coaching and classroom management strategies were provided. Staff #1 acknowledged the behavior and has adjusted their tone of voice. Requirement: A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: When the concerns about staff #1's behaviors in the classroom were brought to leadership's attention, a supervision meeting was held on 1/10/25 to address the concerns. During an unannounced monitoring visit, documentation of completed emergency plan training was not available for Staff #2 and Staff #3. This was CORRECTED ON SITE. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.
