A renewal inspection occurred at the facility. A physical site review was conducted. The fencing in the outdoor play area was not in good repair. As you enter the play area, the fence line to right had pole at the bottom of the fence that was no longer attached. The fence also had other areas that the fence had come lose from the poles. Also, at the poles at the bottom of the fence, screws were facing in towards the children's play area. These areas were identified and addressed with the director at the time of the inspection. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Staff person #1 has not completed the following required pre-service training within 90 days of their date of hire: pediatric first aid/CPR. Requirement: Staff person(s) shall complete professional development in the topics of 3270.31(f)(1 -- 10). Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Correction: Staff #1 completed pediatric first aid and CPR training on 6/30/25. Child #5 had a health report in their file dated 7/1/24 and another dated 6/13/25. The file of child did not have an updated 6-month health report after 7/1/24 but received one on 6/13/25. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: An updated health report for Child #5 was obtained 6/13/25. An unannounced inspection occurred at the facility. A physical site review as conducted. Staff #1 identified a group of 4-children that contained 1-infant and 3-toddlers. Staff #2 identified a group of 6-children that contained 1-toddler and 5-preschoolers. The group at large contained 13 children, which left 3 children unaccounted for in a supervision group. To maintain appropriate supervision, a third staff person would have been required. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff have been informed if children arrive on site causing site to be out of staff:child ratio, the child will not be accepted into the center until another staff member has arrived on site in order to always remain in staff:child ratio compliance. Each staff has been given specific children they are assigned to and have been made aware they must know the names and ages of the children they are responsible for supervising at all times. An unannounced inspection occurred at the facility. A physical site review was conducted. Staff #1 stated that Staff #3 stepped out to run an errand quick. This left Staff #1 and Staff #2 out of ratio for a period of 15-minutes. With Staff #1 and Staff #2 were 1-infant, 2-young toddlers, 2-older toddlers and 8-perschool children. This number of children would require a third staff person. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3270.51--3270.55. A renewal inspection occurred at the facility on 11/14/24. A physical site review was conducted. A mixed-aged group of children was viewed watching a movie and receiving care in the right classroom. Several of these children ranged in age from young toddler to preschool. These children were viewed strapped into infant rocker chairs and lower to the ground toddler chairs. These children were viewed attempting to get out of the chairs but were unable to because of the safety straps being used. A verification visit occurred at the facility on 1/14/25. Child #1 (preschool) was observed belted into a chair for a period of 30-minutes. Child #2 and #3 (both preschool age) were observed belted into chairs for a period of 1-hour. Child #4 (preschool) was observed belted into a high chair for a period of 1-hour and 30-minutes. These children were viewed in the right classroom watching 2-movies. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. On 1/14/25 a verification visit occurred at the facility. At that time, Child #5, who is an infant, was observed an infant seat for 25-minutes. Child #6, who is an infant, was also observed in an infant rocker for 35-minutes. These infants were not provided stimulation from staff as required by regulation. Requirement: Infants and toddlers shall be provided stimulation by being held, rocked, talked to, played with and carried. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. The legal entity must arrange for all facility staff at the time of this inspection to receive a minimum of two hours, in-person PQAS-approved training, regarding Developmentally Appropriate Practices. The legal entity must receive DHS approval of the training content prior to scheduling the training. Correction: No child will be strapped in the chair. All special needs children that require the use of adaptive equipment will have a prescription from their doctor specifying what adaptive equipment can be used and the duration of use for the adaptive equipment. A verification inspection occurred at the facility on 1/14/25. Upon arrival at the facility Staff #5 was the only staff present with a group of 15 children that ranged in ages from infant to preschool in the right room child care space for a period of 1-hour and 30-minutes. Staff #5 was observed leaving this child care space on numerous occasions to go into the middle/main room to answer the door, go to the office and to change children's diapers. Staff #5 also left this space to go to the left room child care space several times to grab children's snacks and drinks. During the time the children were left unsupervised, the children were observed physically fighting, arguing and sitting on top of each other. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Children must be supervised at all times. Correction: Staff will ensure that all children are properly supervised at all times by escorting the kids with them should they have to leave the classroom to enter another part of the daycare and there is no other staff present to supervise the children. Staff #6 is within 90 days of their date of hire, was working unsupervised with a group of children at nap time and did not have completed pediatric first aid/CPR. Regulation requires staff who are with 90 days of their hire date to be supervised by an appropriate staff until they complete the appropriate trainings. An acceptable plan of correction was received on 12/18/24. A verification inspection occurred on 1/14/25. Staff #6 still did not complete pediatric first aid/CPR and it was confirmed by Staff #5 that Staff #6 was continuing to work unsupervised with children.
