Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. During the allocated inspection, Cert Rep was informed that food that has been previously served to children is not discarded and is sent home at parents request so that parents can see what the child ate. Requirement: Food that has been previously served to a person or returned from a table shall be discarded. Correction: Food will be discarded after served. During the allocated inspection, there was a stroller blocking one of the exit doors located outside of the entrance of the infant room. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: Director and owner will move the stroller to another area in the hallway so that stroller is not obstructing the exit. Inspection: Allocated Unannounced Monitoring. Supervised at all times /Staff assigned to specific children (3270.113(a)/3270.113(a)(1))

During the complaint investigation, it was verified that a child was left unsupervised on the playground for approximately 5 minutes. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Teacher responsible for supervision was terminated on 11/8/24 after investigation. Resource teachers will monitor and observed the classroom routinely. Owner and Directors reviewed name to face count protocol with each teacher individually. All staff will also complete a name to face count training using Goddard curriculum on 1/20/25.
