At the time of renewal inspection observed that emergency information was not updated in each child care space. Each classroom had emergency contact forms that were updated by parents more than 6 months ago. Requirement: If emergency information is updated in a master file, it shall be updated accordingly in other facility records. Correction: Copies will be made and distributed to the classroom. No physical punishment (3270.113(b))

During the inspection conducted on May 22, 2025, video footage dated May 19, 2025 was reviewed. The following observations were noted: · In the footage, Staff Person #1 was seen pulling Child #1 roughly by the arm to remove the child from under a slide. Then, escorted the child to the fence. · When Child #1 attempted to scoot away from the fence, Staff #1 again grabbed the child by the arm and dragged the child back to the same area. · Later in the video, as the children were transitioning indoors, Staff #1 was observed once more grabbing Child #1 by the arm. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Staff person #1 was removed from the class, sent home and placed on suspension without pay. On 5/21/25 staff person #1 was terminated from Children Central. At the time of renewal inspection, file of staff person #1, #4, #6, #7, #10, #11, #12 did not include written record that emergency plan training was conducted on an annual basis. Last emergency plan training staff person #1, #4, #6, #7, #10, #11, #12 received on 2/2/24. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. At the time of renewal inspection, file of staff person #1 included a health assessment conducted on 1/31/23 that was not valid since it was conducted more than 24 months ago. file of staff person #2 did not include initial health assessment with the results of initial tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. At the time of renewal inspection, toys (dolls/playhouses) and yellow chairs used by children in Toddlers, Toddlers 2 and Twaddlers rooms were dirty. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. At the time of renewal inspection observed peeling paint on the surface of adult rocking chair in infant room, a few areas of peeling paint near the mirror in Toddlers 2 room. Building Surfaces (3270.76)

At the time of renewal inspection, the doors of the under the sink cabinets in Toddlers and Twaddlers rooms had laminate that was chipped, peeling and damaged. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. At the time of renewal inspection, file of staff person #1 included a staff evaluation that was conducted on 2/2024 more than 12 months ago. File of staff person #10 and #11 did not include staff evaluation. Requirement: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months. File of staff person #8 did not include a current state child abuse and neglect registry check from the state of West Virginia in which staff #8 resided within the last 5 years. Both staff are not a provisional hire. File of facility person #12 included NSOR from 12/23/19 that was more than 60 months old. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person # 5 must be removed from child care position by close of business. NJ child abuse clearance must be obtained before staff person #5 may resume a child care position. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.
