On2/9/26 facility self-reported noncompliance. On 2/8/26 Staff # 1 dragged Child # 1 across the carpet by child's ankles. Resulting in a rug burn on child's lower back. Facility terminated Staff # 1 immediately. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: 1. All staff will take papd registry training # 382908 titled "Stress, Trauma and Mindfulness: Self-care for Educators on 3/23/26. 2. All staff will review the YMCA Staff and Volunteer Child Abuse Prevention Code of Conduct and sign. 3. During renewal inspection on 4/28/25 an infant was observed sleeping in a crib with the pacifier strap attached to clothing. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. It was reported that on 3/12/25 Child # 1 was left unsupervised in the play area for approximately 2 minutes. Staff # 1 admitted leaving Child # 1 unsupervised in the play area. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Tiered LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. Director will update the Supervision Policy; the policy needs to include that staff will use face to name to identify children in their supervision group. Upon reviewing the policy staff shall demonstrate to the director how they are doing the name to face recognition to ensure they understood the policy change. Supervision Policy will need to be approved by Regional Office. Once approved all staff will be trained and verification of training must be on file at facility. The operator shall provide a date for when this will be completed. 3. Compliance with this regulation will be measured with at least two unannounced visits over the next three months. The correction date to complete this portion of the plan shall be at least three months from the date the plan is marked acceptable by the regional office. Correction: 1. Children will be supervised at all times. 2. Name/Face Recognition rules will be added to the Supervision Policy. 3. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group.
