Requirement: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. The above ground pool was observed to be accessible by ladder and a closed but unlocked gate in the outdoor play space. Requirement: An aboveground swimming pool which is not in use must be made inaccessible to children in accordance with the swimming pool barrier guidelines of the United States Consumer Product Safety Commission. Correction: The above ground pool was made inaccessible to children by securing the gate with a lock and restricting access to the pool by raising the ladder. Inspection: Renewal. Clean and good repair (3290.102(a))

A bench in the outdoor play area was observed to be unsturdy and weathered, causing the surface to be rough and a potential splinter hazard. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. English Ivy and Poison Ivy were observed to be growing around the tree where the children play outside. Requirement: Toxic plants are not permitted in a child care space. Upon inspection of children's files, it was found that Child #1 and Child #2 were each missing one of four required pneumococcal vaccinations, as required by the ACIP for their ages. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Correction: The children were suspended from Daycare until they completed their vaccination cycle with the pneumococcal vaccine. Inspection: Renewal. Upon inspection of the physical site areas, it was observed that multiple mats [4+] used for rest equipment were torn at the corners and edges, exposing the internal foam material. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Upon review of each personnel file, it was found that documentation for the initial and annual emergency plan trainings for Facility Person #1 and Facility Person #2 was missing/not completed. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children.
