A layer of dust was observed on the ceiling vent above the diaper changing table in the Monkey classroom. An unidentified brown substance was observed on the toilet seat and on a divider between the toilets in the Lion classroom. Corrected on site by cleaning the areas. A layer of dust was observed on the bathroom vent in the boy's bathroom in the Giraffe classroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Ceiling vents were cleaned with vacuum and wiped down. Surfaces in the bathroom were cleaned during the inspection. Inspection: Renewal. Small Toys and Objects (3270.103)

Plastic bags, holding diaper supplies, were observed to be accessible to children in the diaper changing area in the Monkey classroom. Plastic shopping bags were observed to be accessible in the Owl classroom. Corrected on site by placing bags out of reach of children. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Several shelving units in the Monkey room were observed to have dust accumulated in the corners of the shelves. The lid of the trash can in the Monkey room was observed to have streaks of dried food. Corrected on site by cleaning the trash can lid. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. A trash can with a swing lid was observed in the diaper area of the Monkey classroom, which allowed soiled diapers to come in contact with the lid before dropping into the trash can. The trash can in the Penguin room was covered with a swinging piece of plastic, which allowed soiled diapers to come into contact with the plastic before dropping into the trash can. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: Trash cans have been replaced with foot peddle trash cans. The swinging flap of the trash can was removed. Inspection: Renewal. It was observed that there were 2 staff persons supervising 11 children in the Penguins classroom (young toddlers). The extra child in the room was staff person #2's school-aged child. (corrected during inspection). Requirement: The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3270.51--3270.55. Correction: Staff member is required to take a supervision training. Staff person #1 will properly supervise children at all times.
