At approximately 9:00 AM, 17 children were observed receiving care in the School Age Classroom space that has a designated maximum capacity of 12. Clean and good repair (3270.102(a))

In the Pre K Classroom there was a child sized chair that had ripped padding leaving the wooden frame below exposed creating a rough edge for the children in care. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The wooden chair that is covered with a mesh cover and then a fabric cover was showing a ripped fabric cover. Two tubes of diaper cream labeled, "Keep out of reach of children," were out on the changing table accessible to the children. The facility had a bottle of Acetaminophen in the locked medicine cabinet that did not identify the name of the child for whom the medication was intended. Requirement: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Correction: Bottle of Acetaminophen was immediately removed from medicine cabinet and proper disposed of. According to staff statements, Staff #1 crumpled up Child #1's artwork in the presence of the other children as a consequence for Child #1 ripping up another child's work. Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. In the School Age Classroom, a storage cabinet was observed to be unlocked, allowing 12 sunscreens labeled, "Keep out of reach of children," to be accessible to the children in care. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Magnetic, plastic letters were observed to be less than 1 inch in diameter and accessible to children who are still placing objects in their mouths in the K Prep Classroom. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Staff was informed of this regulation and have been trained on where children who are still placing object in their mouths are allowed to nap/classroom they are allowed to be in, in order to stay compliant. An approximate two-foot section of the baseboard on the wall when you enter the children's restroom was removed leaving dry wall and wood exposed. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Baseboard in restroom has been repaired. Center staff will conduct daily inspections of all areas to ensure there are no cracks or damages throughout the building. At approximately 9:00 AM in the Infant Classroom, Staff #1 was asked to name their primary assigned group of children. Staff #1 stated that they were not currently responsible for any children. Staff #2 named three infants. Staff #3 named four infants. Cert Rep observed 12 children in the childcare space. At this time Staff #4 came back into the childcare space and told Staff #1 that they were responsible for their assigned group of children while they were using the restroom. Staff #1 was asked again if they were responsible for any children. At that time Staff #1 stated they were responsible for the remaining 5 children including two young toddlers and three infants. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Correction: Staff created updated ratio groups and rearranged primary groups to be in compliance with ratio. Teacher will maintain a regular schedule with assigned group of students. Group lanyard will transition to teacher stepping into classroom to ensure that every Teacher know who they are responsible for within the classroom they are assigned.
