Water-stained ceiling tiles were observed at the facility as follows: * 2 in the hallway by the old camera * 2 in the hallway by the Preschool Room Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The 4 stained ceiling tiles (2 by the old camera in hallway, 2 in the hallway by the preschool room) were replaced with new ceiling tiles. Inspection: Renewal. Locked or inaccessible (3270.66(a))

Toxic, "keep out of reach of children" materials were observed to be accessible to children as follows: * White-out and sharpies were located in a low bin by the desk in the PreKdg room (corrected on-site). * Aquafor ointment was observed in an accessible bin by the desk in the Preschool room as well as hand sanitizer and other hand creams in a low cabinet that was not locked (all corrected on-site). Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: During the inspection, the bin containing white-out and sharpies in the Prekdg room, was moved to an area that was not accessible to children. In the Preschool room, the bin containing Aquafor was moved to the cabinet where the hand sanitizer and other creams/ointments were located, and the cabinet was locked. Inspection: Renewal. Staff Person #1's file did not include a high school diploma with 2 years of documented childcare experience OR a high school diploma that includes verification of completion of a post-secondary training curriculum described in §3270.31(c). Therefore, Staff Person #1 was an Aide, working alone with children, unsupervised. Requirement: An aide or a combination of aides shall be supervised at all times by a staff person qualified at minimum as an assistant group supervisor. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. Correction: Staff Person #1 provided their high school diploma and documentation of hours of childcare experience, which are in their staff file. Staff Person #1's file did not include a high school diploma with 2 years of documented childcare experience OR a high school diploma that includes verification of completion of a post-secondary training curriculum described in §3270.31(c). Requirement: An assistant group supervisor shall have a high school diploma or a general education development certificate and have completed a post-secondary training curriculum described in §3270.31(c). An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. Correction: Staff Person #1 provided their high school diploma and documentation of hours of childcare experience, which are in their staff file, that qualifies them as an AGS. The record for staff persons 9 and 10 did not include documentation that the classroom training portion of the curriculum was completed. Staff persons 9 and 10 were observed working as aides during the inspection. Requirement: An individual 16 years of age or older who is enrolled in an approved training curriculum may be used as a staff person if a representative of the training institution certifies in writing that the individual has completed the required classroom training and is currently enrolled in the curriculum. An aide shall have a minimum of an 8th grade education and enrollment in a training curriculum described at §3270.31(c). The classroom training portion of the curriculum shall be completed. Documentation of completion of classroom training and continuing enrollment in the training curriculum shall be included in the staff person's file. Correction: Had Diversified Occupations Program from Cumberland Perry Vo-Tech teacher rewrite staff persons 9 and 10 documentation letters. Staff persons 9 and 10 will be used as volunteer status and not aide status until they gradate high school in May of 2025. Inspection: Renewal. Age and Training (3270.31(e))

Staff persons 1, 2, 4 and 7 did not obtain 12 clock hours of child care training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Correction: Always have staff persons hired prior to December 2020 complete their 12 clock hours of child care training prior to December of that current year. Previously we thought we were in compliance with staff trainings when going off the March-March timeframe since our inspection month is in March. We were not aware of the announcement about staff member's being hired prior to December 2020 and running off of that month as to when trainings were due. Inspection: Renewal. The health assessment on file for Staff person 11 did not include information about a physical examination, examination for communicable diseases and the result of that examination, and information on medical problems that what prohibit a staff from providing adequate care to children. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. The record for Staff Person 3 included NSOR verification dated 1/14/2020, with an update on 1/15/25. NSOR verification was not updated within 60 months as required. The record for staff person 8 included a State Police clearance dated 10/21/24. The State Police clearance for staff person 8 was not obtained within the 45-day provisional timeframe. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Begin to submit NSOR verifications two months in advance to ensure that we get the clearance back in time/the clearance doesn't expire. Health and Safety Training (3270.31(f))

Staff persons 9 and 10 did not complete the following required pre-service training within 90 days of their date of hire (see LIS code sheet): (f) 1-9. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Fire safety - 1 yr. (3270.31(e)(4)(ii))

The record for staff person 7 included documentation of fire safety training on 10/15/23 with an update on 2/22/25. Fire safety training was not completed annually as required. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Correction: Have staff members complete their fire safety training each year during the timeframe that they need to retake the fire safety training. Inspection: Renewal. The records for staff person 7 included documentation of pediatric first aid and CPR training that expired 8/2024. Updated training was not completed until 2/20/25. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: We will make sure that all staff persons have their renewed CPR and first aid training completed prior to the expiration date of the training. Inspection: Renewal.
