On 4/13/26 during a complaint inspection Staff # 1 and # 2 admitted that Child # 1 left the facility. Staff # 2 admitted Child # 1 was gone for at least 20 minutes. Child was found down the street at a grocery store. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Tiered LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all staff to receive a minimum of three hours of training regarding Supervision. The training must be in-person, and outside of childcare hours. The legal entity must receive DHS approval of the training content prior to scheduling the training. This portion of the plan shall have a correction date of when training is complete. 3. The operator shall demonstrate the ability to maintain compliance with supervision of children during all unannounced inspections conducted by the Department over the next three months. This date will be 3 months from the time the corrections are accepted. Correction: Children will be supervised at all times. Building Surfaces (3270.76)

During a complaint inspection on 4/13/26 staff # 3 admitted that the gate in the toddler room was broken on 4/2/26 when Child # 1 left the facility. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The gate that was installed is not a requirement. the room has a door which is fine and in good repair. The gate was removed when it was found to be inoperable. Files for Staff # 1 and # 3 were not in compliance with CPSL regulations. File for Staff # 1 contained an expired mandated reporter training dated 1/26/21. File for Staff # 2 contained an expired NSOR dated 2/8/21. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person # 2 may not work in a childcare position at the facility. The CPSL was revised on 9/18/2018 to include a requirement that all clearances be updated and on file at least every 60 months. Tiered LIS: 1. CPSL regulations will be followed at all times. This portion of the plan must be immediate. 2. The legal entity must attend and arrange for anyone responsible for hiring new staff and maintaining files for current staff to register for and attend the existing provider orientation conducted by the NE Regional Office on 2/25/26. Health and Safety Training (3270.31(f))

Staff person # 3 has not completed the following required pre-service training within 90 days of their date of hire: Health and Safety Topics including pediatric first aid/CPR from a PQAS approved trainer and a PQAS approved curriculum. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10, Health and Safety Topics including pediatric first aid/CPR from a PQAS approved trainer and a PQAS approved curriculum. Staff person shall complete professional development in the topics of 3270.31(f) within 90 days of hire. Staff person # 3 will have until 3/6/26 to complete the required training. Until such time as the required training has been completed, staff person # 3 must be supervised, when interacting with children, by a staff person who has completed the required training related to this citation. If there are no staff available to supervise staff person # 3, staff person # 3 may not work in a child-care position at the facility. Play globe in the school age room was observed to be broken, causing sharp edges. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
