1.) *SERIOUS* Personnel: A child had injuries that were unknown to personnel. Complaint finding: Substantiated (complaint #19716). Plan to correct: Immediate correction: Director spoke with teacher and instructed her to follow the following actions - when you discover a mark or injury assess the child immediately. - notify parent/guardian as soon as injuries are discovered. - Report to director so she can address - concerns & expectations Preventative Plan: Schedule: Training concerning supervision requirements. - conduct health checks - personnel will have 30 days to complete training - staff will be periodically observed Ongoing: Director will monitor staff performance Failure to follow supervision requirements and reporting procedures will result in disciplinary actions. Licensing observed an infant that just turned 3 months old swaddled asleep in the crib. Visit: Follow Up / Full. Plan to correct: Licensing removed swaddle from infant asleep in the crip. Licensing informed personnel infants only under 3 months maybe swaddled. Director is having a meeting in early November to revise safe sleep practice. Inappropriate sleep environment
Licensing observed infant teacher walking down the stairs and into the classroom and the teacher in room exiting. Licensing walked in behind the infant room teacher and observed 2 infants (6 & 9 months) asleep in swings with blankets covering the children. Personnel stated that one infant asleep is swing for maybe 10 minutes. Other infant asleep for maybe 5 minutes. Infant teacher was gone for about 5 minutes to use the restroom. Visit: Follow Up / Full. Plan to correct: Licensing informed infant that children need to be put in crib immediately upon falling asleep. Infant teacher got out the 6 month old first and stated her child was the other child and apologized. Personnel removed both infants out of the swing to appropriate sleep environment. Director is having a meeting in early November to revise safe sleep practice. Director will refresh safe sleep procedure with infant teachers tomorrow. Plan to correct: In requirements to 340-110-3-284 We have gone over licensing requirements regarding the care and education of the children. All staff will participate in requirement training provided by DHS. If an incident such as this should occur again personnel will be: 1. Removed from the classroon. 2. Placed on 30 days probation. 3. Possible dismissal. All staff have been informed if they are upset or frustrated it is much better to call for someone to step in while you take a break than to become frustrated with a child.
