    Licensing observed only one (1) personnel present upon entering the room. Licensing conducted count of 7 children. Licensing asked personnel present youngest age in the room. Personnel stated 1 infant. Visit: Periodic / Full. Plan to correct: When personnel informed licensing the youngest age in the room was 1 infant. There was a knock coming from a door. Personnel excused themselves from licensing to open the door. Licensing observed another individual walk into the room. Licensing asked who individual was and was informed the other teacher in the room. Licensing asked were personnel was and personnel stated was in the bathroom (employee). Licensing informed personnel that ratios must be met at all times and if an individual needs to leave the classroom someone must replace them before leaving. Next in charge will inform director and inform personnel ratios must be met at all times. In the future will maintain licensing requirements. Source: OKDHS Child Care Locator program record.

    Licensing observed Central Detection system was last tagged and inspected on 2/15/2023. Visit: Periodic / Full. Plan to correct: Director will call Pioneer on Friday. Director will try to have Pioneer to schedule for next week, asap. Director will inform licensing when Pioneer is scheduled to come and send a picture of the system tagged and inspected. Licensing observed verification not maintained, personnel having ELCCT within 90 days, and personnel not meeting continuing professional development. Visit: Periodic / Full. Plan to correct: Director will have personnel go in and update information with correct position, make sure all training is correct, if not print completion and turn into the director, and remove individuals no longer employed, personnel will enroll in ELCCT immediately and personnel will have it completed by 10/11/24, personnel needed will apply for PDL by 10/4/24 and in the future personnel will apply for PDL 60 days prior to the PDL expired.
