Licensing observed monthly drills and annual drills not documented. Monthly drills were not conducted for the month of February. Visit: Periodic / Full. Plan to correct: During the visit, Ms. Arevalo took down the old drill log from 2025, and replaced it with a blank drill log for 2026. Ms. Arevalo added the dates for the drills conducted in January. Caregiver records

Licensing observed caregiver's attendance to only have check boxes. The record did not have a name or the hours worked. Visit: Periodic / Full. Plan to correct: During the visit, Licensing printed out attendance sheets with names, times in, and times out for Ms. Arevalo. Licensing also emailed the sheets to Ms. Arevalo. In the future, Ms. Arevalo will put personnel's attendance sheets on a stand in her home where it is easy for her to see. In the future, Ms. Arevalo will have more copies of the forms on file. Ms. Arevalo will have parents fill out the compliance file notification and insurance notification forms annually in November. Primary caregiver and Licensing discussed trainings Primary caregiver can take. Moving forward, Primary Caregiver will complete the required number of training hours needed for her OPDL to be current.
