Facility/Home Environment: Provider's home is unclean and unsanitary. Complaint finding: Substantiated (complaint #19334). Plan to correct: Provider and staff will keep the home clean, cleaning upon spills, kitchen, rooms, etc. As well as sanitizing at the end of each day. Outdoor play safety

One section of the fence is leaning. One board on the fence has come loose & there is a space large enough for a child to fit through. Visit: Periodic / Full. Plan to correct: Provider will supervise the children while outside until the fence can be repaired. Provider will contact his maintenance worker to repair the fence.
