    Evidence of violation: It was found upon arrival at the concern visit that a staff member did not have an approved CBCU memo. Although, provider submitted an approved CBCU memo after this worker left the visit there was an evidence of staff scheduled without an approved memo. Method of correction: Do ensure you have a copy of the approved CBCU memo prior to allowing a staff member to work with children. Time allowed for correction: 20 Day.
