   Correction order — Rule(s) 75-03-10-12 - Minimum qualifications for all staff members responsible for caring for or teaching children. 1. Staff members (g.) Shall not place a child in an environment that would be harmful or dangerous to the child's physic

   Evidence of violation: A child was being changed on a changing table. Staff member is seen on camera turning back and take 2 steps away from the changing table. Staff members hands are not on the infant. The infant rolled off the table while staff members back was turned. This was a very brief moment (2-3 seconds). A major injury occurred to the infant as a result of the fall. Method of correction: Center Director shall conduct a staff training on infant changing and supervision. Training shall consist of reviewing developmental stages of infants and to review supervision while changing. The training shall include reminding staff to Always have all supplies ready on the table, keeps eyes on infant at all times while changing on an elevated surface. Review that is it not just changing tables, but also outdoor play equipment, highchairs, and stairways that need extra staff attention when supervising infants and toddlers. Email a recap of the training to Licensing Specialists and have staff sign a form stating they completed infant supervision training conducted by the center director.
