   7.11(03) — Health

   During a review of one medication and one IHCP. The IHCP was reviewed, it was noted that the plan expired on 8/2024. Corrective action: The medication/IHCP in the Emergency Bag were reviewed during the visit because this child is a student of the 310 site who was visiting the 316 site as part of a transition process as this child is aging up in to preschool and moving to our 316 site. The child's file was not reviewed during the licensing visit. However, there is documentation from medical provider in the child's file that the plan changed on 7/19/24 to a less severe medical status but this information from the provider was not enough to renew the IHCP so we had been working with the parent to properly update. We had communications to the parent in writing on 6/11/2024, 9/5/2024, 11/19/2024, and then again on 1/24/25 documented in the file in an effort to obtain proper IHCP information. Other verbal communication was also made to the parent to request the appropriate paperwork for the new plan. Prior to enrolling the family had moved from outside of the Western Mass area and was in process of establishing medical providers in this area. Appointments were set and then changed during this time period We have documentation of that as well. As of 1/29/25 we have everything we need documented in the file & emergency bag to represent new IHCP.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
