The stairs of the back exit were covered with ice due to a snowstorm. The exits used as escape routes need to be kept clear of ice and snow. Corrective action: Scraped ice from stairs and it’s clear to use.. Regulation: The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. The children's records were missing the following: 1.) Some of the records were missing the date of admission 2.)Four of the records were missing proof of lead exams. 3.) Two of the records were missing an annual update form. 4.) One record was missing page 1, 2 and 3 of the enrollment record. Corrective action: I updated [redacted] enrollment package. Added 1~2~3 pages. I updated the annual form for [redacted]. I have Excel’s lead test & [redacted] up to date on file. I’m waiting for the lead test for [redacted] have an appt April 2nd. The water temperature was exceeding 120 degrees F. It was set to 121 degrees F. The water temperature must be adjusted to not exceed 120 degrees F. The dog leash needs to be removed from the side exit. Discussed the swing structures and slides can not be used during day care hours. The structures do not meet the required fall zones. Corrective action: I put the water temp 119 like licensor told me. I took the dog leash out the way. I also moved the swing sets and it will not be used during daycare hours at all.. Regulation: Outdoor Space. The licensee must maintain, or have access to, an outdoor play area of at least 75 square feet per child who is outside at any one time., Indoor Space.
