The documentation for staff meetings did not include the start and end time of each meting making it difficult to assess compliance with the required 2 hours of staff meeting time her month. Corrective action: We have provided the times and attendance of staff for the meetings. In the future, times and staff that attended will be added to meeting notes. On the day of the visit the method of attendance did not provide for the moving of children from one group to another. It also did not allow for educators to now which children were on the premises at any point of the program day. Corrective action: The staff have met and have developed a paper attendance form to use in addition to the Brightwheel app to provide for the moving of children from one group to another. A blank copy of the form, has been added to the document library.. Posted Building and Fire Inspections were expired on the day of the visit. Corrective action: Current inspections notices were printed and posted in the building. Updated inspections were posted to the LEAD document library on 4/16/2026. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). Three children diagnosed with chronic medical conditions did not have current IHCP on file on the day of the visit. Corrective action: We have sent out the EEC IHCP form to children with chronic medical conditions. We have added the completed ones to the children's files.. Regulation: Individual Health Care Plans The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. Random review of children’s records revealed that the program did not have a transportation plans on file for all children. Corrective action: We have printed the The Commonwealth of Massachusetts EEC Small and Large Group Transportation Plan and Authorization form and have had the parents sign it. This was placed in each child's records along with Pioneer's Transportation plan and authorization form.
