On 12/8/2025, five children, between three and five years of age, were not adequately supervised in the Center Room when the Program Director was present in the facility's kitchen for approximately one minute with no other staff members present in the unit. On 12/8/2025, one preschool aged child was not adequately supervised when they entered the Center Room unaccompanied. No staff members were present in the Center Room while the child remained in the unit for approximately one minute.
