HAR §17-892.2-3 — Background checks according with chapter 17-801 (criminal history, sex offender registry, child abuse/neglect, adult abuse perpetrator, employment history check, and other information)

Please complete the following information on the form DHS 959A: the date EH mailed and the date EH returned. Keep copies of all forms mailed out to former employers. If EH forms are not returned after the first try, mail out the again in a few weeks and log the date of the second mailing. HAR §17-892.1-17 — Health and safety training

§17-892.2-35 Staff training, experience, and personal qualifications. (c) Each caregiver, substitute, and volunteer within forty-five days of hire, shall complete initial training that is approved by the department in health and safety standards . . . . The health and safety training for XXXXX has expired. XXXXX reported on 02/24/25 that trainings were submitted to PATCH. Once XXXXX health and safety training is updated in the system, this deficiency will be satisfied and we can issue a regular license. Correction: The Worker verified that staff member XXXXX has completed her on-going health and safety trainings, and submitted them to PATCH.

HAR §17-892.1-3 — Criminal history check

Please submit a new DHS 948 Authorization for Background Check and to Release Findings for XXXXX. We are also awaiting the criminal history check clearance results for XXXXX. Once the clearance form for XXXXX is received and the clearance for XXXXX is complete, this deficiency will be satisfied. Correction: XXXXX cleared in HANA on 02/04/25 and it was reported that staff memberXXXXX is no longer with the facility effective 01/31/25.

HAR §17-892.1-3 — Child abuse/neglect check

Please submit a new DHS 948 Authorization for Background Check and to Release Findings for XXXXX. We are also awaiting the child abuse/neglect check clearance results for XXXXX. Once the clearance form for XXXXX is received and the clearance for XXXXX is complete, this deficiency will be satisfied.
