Please submit a copy of the 2024 annual Food Inspection Report and the green placard that was issued by the Department of Health. To request a copy of the food inspection report, please contact doh.foodsafetyoahu@doh.hawaii.gov. Correction: Received a green Placard and Food Establishment Inspection Report from the Department of Health, both dated February 13, 2024.
