HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Annual health and safety training has expired for the following employee(s): XXXXX Please submit their training certificates to PATCH so that the training hours can be recorded in the system. Reminder: all caregiving staff need at least 16 hours of department approved training on an ANNUAL basis in at least two topic areas and substitutes need at least 10 hours of training. If they do not complete this requirement by February 28, 2026, we will have to exclude the staff member(s) and they will not be able to count towards the staff-child ratio until they meet training requirements, which may affect your license to operate a Group Child Care Center (GCC). Correction: All staff meet this requirement.

HAR §17-892.1-37 — Sanitation inspection

Please submit a current (not expired) Food Establishment Permit for the facility. Note: Because of a Department Health clerical problem in processing the permits timely, DHS is accepting the following items in lieu of an actual Food Establishment Permit at this time: 1. A copy of the Food Establishment Permit Inspection Report. 2. A copy of the Green Placard "Pass/Fail". Please submit both items. NOTES: 1. The last permit DHS has on record of expired on 8-2-24. 2. CCLU1 received a Green "Pass" Placard and Food Establishment Permit Inspection Report dated 5-18-23. However, that pertains to last years inspection. Please submit the Placard AND inspection report from the 2024 Food Establishment Permit inspection. Please contact the Department of Health (DOH) at (808) 586-8000 or doh.foodsafetyoahu@doh.hawaii.gov to request a copy of the inspection for your food establishment permit. If you did not have an inspection, please request one through the DOH. Correction: Worker received copies of the following: --Copy of a satisfactory Food Establishment Permit Inspection Report dated 12-4-24. --Copy of a Dept. of Health (DOH) Green Placard showing "PASS" dated 12-4-24.

HAR §17-892.1-3 — Child abuse/neglect check

We received a completed annual clearance form for XXXXX and it is processing. --Two staff members; XXXXX have clearances that are scheduled to expire within the next 2 months. Please have them submit a completed DHS 948 Authorization for Background Check and to Release Information form. Once we receive the completed forms, we can process their annual clearance checks. Once their checks are satisfactory, this deficiency will be corrected. Correction: 12-11-24: --XXXXX annual clearances are complete and is eligible in the system. --Completed annual clearances forms for XXXXX and XXXXX have been received by Child care Licensing Unit 1, and submitted for processing.

HAR §17-892.1-17 — All caregivers meet qualifications

Staff member XXXXX is not registered as a caregiver with the Hawaii DHS Early Childhood Registry. Please contact PATCH for registry information. Correction: Child Care Licensing (CCL) Worker R. Miller confirmed that XXXXX now holds a current Aide qualification under Hawaii DHS Early Childhood Registry.

HAR §17-892.1-22 — One caregiver with current certificates during operating hours

Worker verified Heartsaver Pediatric First Aid, Child/Infant CPR AED, Adult CPR AED Certificates for 4 staff. Please submit certifications for these two additional staff members: --XXXXX --XXXXX Correction: The CCL worker received the following certifications: -- American Safety & Health Institute (ASHI) Medic First Aid certification for XXXXX: Pediatric First Aid/CPR for Child/Infant/Adult exp 03-2025 --American Heart Association (AHA) Heartsaver Pediatric First Aide CPR Certification for XXXXX for Child/Infant /Adult CPR which expires on 11/2026.

HAR §17-892.1-29 — Physical exam clearance

Please submit evidence of a satisfactory employment physical examination taken within 12 months of date of hire for staff member: XXXXX. Correction: 12-11-24: Received email from Kamaaina Kid's Director's XXXXX and XXXXX stating that XXXXX was a transfer with no break in coverage from Kamaaina Kids King Street location where she was vetted and submitted proof of TB test result and physical examination.
