HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Director to submit completed certificates to PATCH to satisfy trainings hours for three staff members. Correction: Director resent certificates to PATCH on 12/12/2026.

HAR §17-892.2-63 — Furniture, equipment and toys shall be sturdily constructed with minimal hazards

Director to repair the chains to the swing set. Correction: Chains on the swing set has been replaced.

HAR §17-892.2-41 — Health record on each child

Director to submit completed physical exam for one child as discussed. Correction: Worker reviewed child's record and is now complete.

HAR §17-892.2-61 — Facility shall complete practice drills for various emergency situations at regular intervals, or whenever a new procedure has been added

Director to submit log of completed practice drill for other emergency situations. Correction: Worker verified practice drill completed.

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

Director to contact MFD for an annual fire inspection of the facility. Fire extinguisher to be re-serviced in January 2026. Correction: Fire inspection report dated 01/20/26 received and on file.

HAR §17-892.1-33 — No exposure to lead based paint

Director to repair (rust) or replace the chain link on the swings. Correction: The top chain link on the swings have been replaced with new links. Per director's email on 01/27/2025, the links have been thread glued to keep it tight from unscrewing.

HAR §17-892.1-6 — Insurance coverage policy

Director to submit a copy of a current Certificate of Liability Insurance. Correction: Current liability insurance expires February 2025. Director to submit new certificate when renewed.

HAR §17-892.1-26 — Annual nutrition review

Director to submit a current annual nutrition review report. Correction: Annual nutrition review completed and on file.

HAR §17-892.1-31 — Written evidence of fire warning device inspected initially and annually by fire inspector. report dated:

Director to contact MFD for an annual fire inspection of the facility (satisfactory report) and have the fire extinguishers re-serviced. Correction: Fire extinguisher serviced in January 2025.

HAR §17-892.1-36 — Sanitation inspection

Director to submit copy of their renewed food establishment permit from the Department of Health (current one expires 01/24/25). Correction: Received Food permit expires 01/24/2026 (on file).
