HAR §17-892.2-35 — All caregivers meet qualifications

The preschool is licensed for 69 children, ages over 2 years old to under 6 years old. On February 2, 2026, the worker observed the following groups of children and staff: 2 Director/Teacher, 1 Teacher qualified, 6-Aides Aide in classroom was not teacher qualified.

HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Staff did not have current training hours.

HAR §17-892.2-36 — Staff-child ratio and maximum group size shall be implementedmet and maintained during all hours of operation

The preschool is licensed for 69 children, ages over 2 years to under 6 years old. On February 12, 2026, the worker observed the following groups of children and staff, not meeting the staff-child ratio requirments: Classroom 2 (Yellow Door) 1 Teacher and 1 Aide (3 year old) Classroom 3 (Green Door) 1 Aide (4-5 year old) Classroom 4 (Purple Door) 1 Teacher and 1 Aide (2 year old)

HAR §17-892.2-36 — Minimum staff employment sequence must be met

Aide in classroom for 4-5 years old was not Teacher qualified.

HAR §17-892.1-3 — Initial/annual Criminal history check

Director shared that she will be submitting 948 for staff who did not complete the annual history check. Correction: Received 948 for staff member.
