   The Worker reviewed staff records and found one (1) staff member, XXXXX, out of fifteen (15) having current pediatric first aid and child CPR certification. Please provide the Worker a current DHS 957 Staff Sequence form for the Group Child Care Center to verify that all hours of child care are covered by at least one (1) staff member with current certifications. If there are additional staff who have current department-approved pediatric first aid and child CPR certification, please provide a copy of the certification to the Worker. Correction: On September 30, 2025, the Worker received the DHS 957 Staff Sequence form and verified that all hours of child care are covered by at least one (1) staff member with current certifications. The Worker also received an additional American Heart Association Heartsaver First Aid CPR AED certification for XXXXX which includes child CPR AED and infant CPR. The certification will expire January 2027.

   HAR §17-892.1-30 — Written health policies for staff

   Please make the following changes to the Staff Handbook's health policies. Smoking is not permitted on campus. (Staff Handbook, pg. 46) - Please include in the Staff Handbook a policy which includes that smoking includes electronic smoking devices. Please also include that personal nicotine products shall be stored out of reach of children. Alcoholic beverages are not permitted on premises. (Staff Handbook, pg. 46) - Please include in the Staff Handbook a policy which includes that alcoholic beverages shall not be consumed prior to and during the hours of operations at the child care facility. Please include in the Staff Handbook a policy which states that medication that impairs or limits staff's ability to provide care shall not be consumed prior to or during the hours of care. Correction: The Worker received an amended Staff Handbook with satisfactory smoking, alcohol, and medication policy under the Smoking, Alcohol, and Drugs/Medication section on page 44.

   HAR §17-892.1-31 — Written plan approved by fire inspector, health consultant or Red Cross

   §17-892.2-61 (e) states that the facility shall make the written procedures for disaster and emergencies available for review by or provide a copy of the plan to each family whose child is enrolled at the child care facility. The disaster plan procedures in the Family Handbook does not sufficiently provide procedures for the disasters including fire, flood, tsunami, hurricane/tropical storm, and earthquake, nor the additional written procedures such as evacuation, and accommodations. Please provide the Worker an amended Family Handbook or provide a separate document, which is given to enrolled families, with the comprehensive procedures. The written procedures for families must include the natural disasters stated above (fire, flood, tsunami, hurricane/tropical storm, earthquake). The plan must also include evacuation (evacuation site), accommodation of toddlers during an emergency, and providing for the needs of children with special needs, disabilities, and chronic medical conditions during an emergency. We recommend including the details from the Staff Handbook's disaster plan in the Family Handbook. Correction: The Worker received the updated Parent Handbook with the requested changes made.
