    According to §17-892.2-11 Statement of Operation Policies (a) Written policies shall be available to parents or guardians which shall include the following: (18) Grievances - the department's information for parents should they want to file a complaint. Please provide the office name and phone number minimally for use at Salt Lake Pre-Plus Head Start. Our office information is: Child Care Licensing Unit 1 677 Queen Street Suite 400A Honolulu, Hawaii 96813 (808) 587-5266 As a reminder, our office information for grievances should only be used for licensed facilities within jurisdiction of our unit. The Worker has received a copy of the Notes page which lists the phone numbers of both Child Care Licensing units on Oahu. Please provide additional context, which shall minimally include that the phone numbers are for parents should they wish to file a complaint. Please also provide verification to the Worker as to how the parents have access to this information, such as posted on the Parent Board in the Salt Lake Pre-Plus Head Start classroom. Correction: The Worker received a photo of the grievance policy with the department's information on the Parent Board of the Salt Lake Pre-Plus Head Start classroom.
