HAR §17-892.1-6 — Compliance with Hawaii Administrative Rules (HAR), Title 17, Chapter 892.2

According to §17-892.2-11 Statement of Operation Policies (a) Written policies shall be available to parents or guardians which shall include the following: (18) Grievances - the department's information for parents should they want to file a complaint. Please provide the office name and phone number minimally for use at Salt Lake Pre-Plus Head Start. Our office information is: Child Care Licensing Unit 1 677 Queen Street Suite 400A Honolulu, Hawaii 96813 (808) 587-5266 As a reminder, our office information for grievances should only be used for licensed facilities within jurisdiction of our unit. The Worker has received a copy of the Notes page which lists the phone numbers of both Child Care Licensing units on Oahu. Please provide additional context, which shall minimally include that the phone numbers are for parents should they wish to file a complaint. Please also provide verification to the Worker as to how the parents have access to this information, such as posted on the Parent Board in the Salt Lake Pre-Plus Head Start classroom. Correction: The Worker received a photo of the grievance policy with the department's information on the Parent Board of the Salt Lake Pre-Plus Head Start classroom.

Please update the disaster plan to meet the new requirements of §17-892.2-61 Disaster Plan for Emergencies (a) The plan shall include the following: (8) Providing for the needs of children with disabilities and children with special needs. (9) Providing for the needs of children with chronic medical conditions. Please provide the Worker a copy of the amended disaster plan. Correction: The Worker received a Natural Disaster Plan which includes that children with disabilities, special needs, or chronic medical conditions will be accommodated according to the details outlined in their medical conditions form.

HAR §17-892.1-31 — Written evidence of fire warning device inspected initially and annually by fire inspector. report dated:

Worker received the fire Inspection Report for Salt Lake Elementary School dated December 14, 2023 and found that the fire inspection report is not satisfactory. This requirement can only be met with a satisfactory fire inspection report. Please provide a satisfactory fire inspection report to satisfy this requirement. Correction: Worker received a satisfactory Fire Inspection Report completed on January 16, 2025.
