HAR §17-892.1-17 — All caregivers meet qualifications

Please correct the following deficiencies per §17-892.2-35 Staff training, experience, and personal qualifications: 1. During our visit on 09/19/25, Worker observed two regular staff in classroom C6, both with aide qualifications. We understand that you have sent documents in to People Attentive to Children (PATCH) for XXXXX's teacher qualification. Once it is recorded in the system that the staff member is qualified, this deficiency will be satisfied. 2. Please submit health and safety training certificates to PATCH for XXXXX as their training period has expired. We understand that XXXXX is primarily located at another site. Please submit a DHS 974A with start date and specific site they are assigned to, and we can switch their association. Correction: Worker verified that all staff have current health and safety training hours as of 12/17/25.

HAR §17-892.1-18 — Staff-child ratio

Worker verified that there were fourteen (14) children in attendance out of sixteen (16) enrolled in classroom C6, with two (2) aides, which does not meet staff sequence requirements. We understand that you have sent documents in to People Attentive to Children (PATCH) for XXXXX's teacher qualification. Once it is recorded in the system that the staff member is qualified, this deficiency will be satisfied. Correction: Worker verified that staff have proper qualifications registered with PATCH as of 12/17/25.

HAR §17-892.1-37 — Sanitation inspection

Please provide a copy of the current Food Establishment Permit allowing food being serve in the classroom. Correction: State of Hawaii Department of Health Food Establishment Permit XXXXX with risk category 3 expires on September 13, 2024. A current DOH Food Establishment Inspection Report XXXXX was completed on October 1, 2024 with routine inspection and green placard issued posted in the service room.
