    Please correct the following deficiencies per §17-892.2-35 Staff training, experience, and personal qualifications: 1. During our visit on 09/19/25, Worker observed two regular staff in classroom C6, both with aide qualifications. We understand that you have sent documents in to People Attentive to Children (PATCH) for XXXXX's teacher qualification. Once it is recorded in the system that the staff member is qualified, this deficiency will be satisfied. 2. Please submit health and safety training certificates to PATCH for XXXXX as their training period has expired. We understand that XXXXX is primarily located at another site. Please submit a DHS 974A with start date and specific site they are assigned to, and we can switch their association. Correction: Worker verified that all staff have current health and safety training hours as of 12/17/25.
