HAR §17-892.2-15 — Name, address, and phone numbers of person to assume responsibility when parents/guardians are not available

Please include the following in the Authorization for Release - Emergency Contact Form: An area where the address of the emergency contacts can be inputted. HAR §17-892.1-33 — Outdoor area shall be fenced or have natural barriers

§17-892.2-63(3) Environmental hazards. The outdoor space shall be fenced or shall have natural barriers to deter children from getting into unsafe areas *Workers observed the play area directly outside of the Head Start classroom is not fully enclosed by a fence or natural barriers. Until this area is enclosed, there shall be no outdoor play. * Please provide us with a plan as to how you will be addressing this deficiency. Correction: Worker verified that the play area outside of Head Start classroom is enclosed by plastic netting fence, which appears sturdy and adequate.

HAR §17-892.1-11 — List of staff members and position

§17-892.2-17(a)(5) Information and records on facility. The facility shall maintain, and make available to the department current records and information including: (5) A list of staff members including each staff member's training, position or title, experience, and health records. Please provide current DHS 954 staff and volunteer listing (attached), including any long term substitutes being used. Correction: Worker received current and complete DHS 954 staff listing via email on 6/6/25.

HAR §17-892.1-31 — Written evidence of fire warning device inspected initially and annually by fire inspector. report dated:

§17-892.2-61(g) Disaster plan for emergencies. Each child care facility shall install an underwriters laboratory listed fire warning device or system in each facility. Written evidence that the device or system has been inspected and approved by a fire inspector... shall be current and on file at the facility. Worker received an unsatisfactory fire inspection report for Kapalama Elementary School. Please provide a current satisfactory fire inspection report. Correction: On 6/16/2025, Worker received corrected satisfactory fire inspection report, with corrections dated 12/20/2024 and initialed XXXXX by firefighter (FF) 3, satisfying this requirement.

HAR §17-892.1-37 — Sanitation inspection

§17-892.2-67(2) Food protection. Food protection policies shall comply with accepted practices of local sanitary codes in chapter 11-50. Workers observed that the food establishment permit for the Head Start classroom is expired. We understand you have called the Department of Health (DOH) to arrange an inspection. Once this is conducted, please send in current unexpired food inspection report and green pass placard. Please also provide Kapalama Elementary School cafeteria's food establishment permit if you are obtaining the meals from the school. Correction: Worker received current satisfactory food inspection report and green pass placard for both Kapalama Head Start and Kapalama Elementary cafeteria via email.
