HAR §17-896-16 — Health and safety training

17-896.1-35 Staff training, experience, and personal qualifications. (b) Each caregiver, substitute, and volunteer within forty-five days of hire, shall complete initial training that is approved by the department in health and safety standards. XXXXX and XXXXX were hired by HFCA on March 13, 2025. Their initial health and safety training assessment shall be submitted to People Attentive to Children (PATCH) prior to April 27, 2025 to be in compliance with the Hawaii Administrative Rules (HAR). Enclosed you will find the Health and Safety Assessment along with the Addendum to the Health and Safety Practices. Correction: Heath and safety training is current for all staff members.

HAR §17-896-6 — Sanitation practices

§17-896.1-11 Statement of operation policies (18) Sanitation practices. Please include in your HFCA BASC Parent Handbook your program's sanitation practices. Correction: Per Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook p. 10, Sanitation, received on 03/03/25.

HAR §17-896-6 — Administering medication

§17-896.1-11 Statement of operation policies (17) Administering medication. Please include in your Before After School Care (BASC) Parent Handbook whether or not Holy Family Catholic Academy (HFCA) BASC administers both prescription (Rx) and over-the-counter (OTC) medication. Correction: Per Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook p. 3, Administration of Prescribed Medications received on 03/03/25.

HAR §17-896-6 — Suspension/expulsion policy

§17-896.1-11 Statement of operation policies (22) Suspension and expulsion of children which includes the following: (A) The conditions under which a child may be suspended or expelled, if applicable; (B) Sufficient timeframe before suspension or expulsion occurs to enable parents to make alternative child care arrangements or to take the necessary action to allow the child to remain enrolled, except as specified in paragraph (22)(C) below; (C) Conditions that may warrant immediate expulsion such as imminent danger to the health, welfare, or safety of the children; (D) Parents and guardians are provided written notification of any concerns that could lead to the child's expulsion; (E) When expulsion occurs, the provider shall maintain a record of the conditions, parental notification, and action taken. Worker received the updated Holy Family Catholic Academy (HFCA) Before & Afterschool Care (BASC) Parent and Student Handbook. The suspension/expulsion policy is included on page 4 and 5, however, it does not cover any of the requirements listed above, see §17-896.1-11 Statement of operation policies (22). Please be specific about what conditions will cause a child to be immediately suspended or expelled; what timeframe so the parents can plan if child is suspended/expelled or take action to allow continued enrollment; what conditions warrant immediate expulsion, including providing written notification of any concerns that could lead to the child's expulsion; and when expulsion occurs, ensure that records are kept regarding conditions, parental notification, and action(s) taken. Correction: We received the updated BSC ASC Handbook on 05/14/25 which meets all licensing requirements, including the suspension/expulsion policy.

HAR §17-896-6 — Mandated reporting

§17-896.1-11 Statement of operation policies (21) Mandated reporting of suspected child abuse or neglect in accordance with chapter 350, HRS. Worker received the Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook on 03/03/25. As previously discussed on 02/27/25, please include in your Mandated Reporters policy on page 6, that HFCA and it's staff members are mandated reporters of suspected child abuse or neglect in accordance with chapter 350, HRS. Correction: Worker received the updated parent handbook which includes that Holy Family Catholic Academy are mandated reporters of suspected child abuse or neglect, p. 6.

HAR §17-896-28 — Personal health habits of staff

§17-896-1-56 Personal health habits of staff. The personal health habits of all caregivers shall not interfere with the protection of the health of the children as follows: (2) No smoking, including electronic smoking devices, is allowed on the premises during child care hours and operation, and personal nicotine products shall be stored out of reach of children; (4) Medication that impairs or limits the staff's ability to provide care shall not be consumed prior to or during the hours of child care. Please include in your Faculty/Staff Handbook that there shall be no smoking, including electronic smoking devices, and personal nicotine products shall be stored out of reach of children, and include that any medication that impairs or limits the staff's ability to provide care shall not be consumed prior to or during the hours of child care, and provide us with your Faculty/Staff Handbook. These requirements can also be included in your Holy Family Catholic Academy Before & Afterschool Parent and Student Handbook as well. Whatever you decide, please provide us with an updated handbook which includes #2 and #4 of Hawaii Administrative Rules (HAR) §17-896-1-56 Personal health habits of staff. Correction: Worker received the updated parent handbook which includes the personal health habits of staff (all above referenced requirements), p. 6.

HAR §17-896-30 — Infection Control

§17-896.1-62 Accidental injury precautions. (b) Procedures for infection control, to use in all situations to prevent the transmission of bloodborne germs that may spread through blood and body fluids that might contain blood shall be in writing. Please include in your HFCA Faculty/Staff Handbook the procedures for infection control (bloodborne pathogen procedures). Correction: Worker received the Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook on 03/03/25, p. 10, Bloodborne Pathogens, which states, "All staff members are trained annually in bloodborne pathogens in accordance with OSHA policies and procedures. Personal Protective Equipment (PPE), such as eye protection, gloves, shields, and masks, is accessible and available if needed. Contaminated items are properly disposed of using biohazard waste containers or bags. Contaminated areas are cleaned with an EPA-approved disinfectant."

HAR §17-896-16 — Health and safety training

§17-896.1-35 Staff training, experience, and personal qualifications. (b) Each caregiver, substitute, and volunteer within forty-five (45) days of hire, shall complete initial training that is approved by the department in health and safety standards which addresses the following areas: (1) - (11). (c) When a caregiver, substitute, or volunteer has not provided child care for one hundred eighty consecutive days or more, the initial training in subsection (b) shall be repeated. The following staff members needs to complete initial health and safety training. We have enclosed the health and safety assessment. Please complete and send to PATCH. XXXXX Correction: Health and safety trainings are current for all staff members.

HAR §17-896-6 — Grievances

§17-896.1-11 Statement of operation policies (20) Grievances which shall include the department's information for parents to file a complaint. Worker received the Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook on 03/03/25. The telephone number for the Child Care Licensing Unit 1, (808) 587-5266, needs to be included in the Grievance Policy on page 10. Once the telephone number is added to the handbook, this deficiency will be satisfied. Correction: Worker received the updated Parent Handbook via email on 04/22/25, which includes the Child Care Licensing Unit 1 telephone number, p. 10.

HAR §17-896-6 — Transportation arrangements

Please include your program's transportation arrangements (if any) in your BASC Program Handbook. Correction: Per Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook received on 03/03/25, p. 6, Transportation, "HFCA BSC/ASC does not provide transportation."

HAR §17-896-21 — One caregiver with current certificates during operating hours

Although the worker has verified two (2) staff members with current child cardiopulmonary (CPR) and pediatric first aid certification, please provide us with additional child CPR/pediatric first aid certifications for staff members so we may ensure that all operational hours are covered during before and after school care hours, in the event that both of these staff members are not at work. To ensure coverage of all operational hours, providing certification of school administrators who are available during those hours would help to ensure this requirement is met. Additionally, the department recommends that all staff members have child CPR and pediatric first aid certifications. Correction: Worker received additional child CPR/pediatric first aid certifications which cover all operational hours for the BASC program. DHS strongly recommends that all BASC staff members be pediatric first aid/child CPR certified.

HAR §17-896-9 — Parent/guardian contact information

Provide us with the Holy Family Health Information Form including Medical and Emergency Information. Correction: Parent/guardian contact information is provided on the Holy Family Catholic Academy Health Room Information Card received on 03/03/25.

HAR §17-896-9 — Child name, DOB and address

Provide us with the Holy Family Health Information Form including Medical and Emergency Information. Correction: Child name, DOB and address is provided on the Holy Family Catholic Academy Health Room Information Card received on 01/24/25.

HAR §17-896-9 — Admission policies that require sufficient information and instruction to be received from parents or guardians that enable caregiver to care the child

Provide us with the Holy Family Health Information Form including Medical and Emergency Information. The form is required to have admission policies that require sufficient information and instruction to be received from parents/guardians that enable caregiver to care for the child: Child's name, date of birth and address; Parent/guardian name and address; Parent/guardian contact information; Name, address and phone numbers of person to assume responsibility when parents/guardians are not available; Name, address and phone numbers of person authorized to take child from facility. HAR §17-896-9 — Parent/guardian name and address

Holy Family Health Information Form including Medical and Emergency Information form was received on 03/03/25. Please include the addresses (separate from the child's information) of each parent/guardian, in the event that parents/guardians do not reside at the same address. Correction: Worker verified that the HFCA Online Application Packet has separate home addresses for the parents/guardians under Household 2.

HAR §17-896-6 — Policies and plans for emergency care

Please include in your BASC Parent Handbook under Emergency Medical Care that an adult will accompany a child to the source of emergency care and shall stay with the child until the parent or parent's designee assumes responsibility for the child's care. Also include the nearest emergency medical facility. Correction: Per Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook received on 03/03/25, p. 3, under Emergency Medical Care, "If a situation arises in which a child requires emergency medical treatment, BSC and ASC staff members will render first aid within the scope of their training and then wait with the childuntil a parent/guardian or 911 help arrives. A staff member will accompany the child to the medical facility and will remain with the child until a parent or parent's designee assumes responsibility for the child's care. Geographically, Kaiser Moanalua and Pali Momi are the closest, however, the Emergency Medical Team will decide where the child will be transported."

HAR §17-896-6 — Maximum number of children permitted by license

HAR §17-896-6 — Ages of children accepted

HAR §17-896-6 — Insurance coverage policy

Please include your insurance coverage policy information in your BASC Program Handbook. Worker verified certificate of liability insurance from The Catholic Mutual Relief Society of America, General Liability, coverage effective date of 07/01/24 to 07/01/25. Correction: Per Holy Family Catholic Academy Before & Afterschool Care Parent and Student Handbook received on 03/03/25, p. 10, Liability, "HFCA obtains liability insurance through The Catholic Mutual Relief Society of America through the Diocese of Honolulu."

HAR §17-896-29 — Written evidence of fire warning device inspected initially and annually by fire inspector, report dated: <date>

Provide us with a satisfactory fire inspection report as there are deficiencies that need correction after HFD's inspection visit on 12/18/24. Correction: Worker received satisfactory fire inspection report received on 01/30/25 with corrections verified by Honolulu Fire Department (HFD) on 01/30/25.

HAR §17-896-29 — Plan practiced at regular intervals

Per Director, plan is practiced monthly. Please provide us with your emergency preparedness log. Correction: Worker received the Fire Exit Drill and Fire Alarm Test Log received on 01/30/25 for Holy Family Catholic Academy which is practiced monthly. Last fire drill was conducted on 01/23/25 at 8:55 a.m.

HAR §17-896-29 — Written plan approved by fire inspector, health consultant or red cross

Provide us with your disaster plan for emergencies (§17-896.1-61) which covers fire, flood, earthquake, tsunami, hurricane, evacuation, relocation, sheltering in place, lock-down, communication and reunification with families during and after an emergency, continuity of operations during and after an emergency; providing for the needs of children with disabilities and children with special needs during an emergency; and providing for the needs of children with chronic medical conditions during an emergency.
